We are looking to appoint an Advanced Nurse Practitioner to join our busy GP Practice which cover 2 sites with a patient list size of over 10,000 patients.
We are looking for an Advanced Nurse Practitioner who is motivated, personable, self-driven, dedicated to providing holistic care and an autonomous Practitioner.
Main duties of the job
Acting within their professional boundaries, the post holder will provide care for patients within the practice and home visits from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision making and expert care, including assessment and diagnostic skills. The post holder will demonstrate critical thinking in the clinical decision making process. They will work collaboratively with the General Practice Team and Chester le street Primary Care Network to meet the needs of patients, supporting the delivery of policy and procedures.

Manage patients presenting with a range of acute and chronic medical conditions, providing expert advice to patients
Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
Request, process and interpret pathology results as required
Refer patients to secondary/ specialist care as required
Maintain accurate clinical records, ensuring read codes are used effectively
Support the team in dealing with clinical emergencies
Recognise, assess and refer patients presenting with mental health needs
Contribute to practice targets (QOF, Prescribing schemes etc.) complying with local and regional guidance
Liaise with external services/ agencies to ensure the patient is supported appropriately (vulnerable patients etc)
Delegate clinical responsibilities appropriately (ensuring safe practice and task is within the scope of the individual)
Support the clinical team with all safeguarding matters, in accordance with local and national policies
Develop, implement and embed health promotion and well being programmes, delivering opportunistic health promotion where appropriate
