Job summary

Outwood Park Medical Centre is a large GP Practice with a friendly supportive team of clinicians and administrative staff.

We are looking to appoint an experienced practice nurse who has worked in primary care and managed patients with long term conditions.

This is a part time permanent position up to 25 hours per week

Main duties of the job

The main focus of the role is management of patients with long term conditions and general nursing interventions for all patients.

Ideally you will have knowledge and experience of COPD and Asthma Management, Diabetes Management, Hypertension, Family Planning/contraception, Cervical Cytology, Childhood Immunisations and Vaccines including travel health.

Applicants must be able to work both independently and as part of a team. They will need to be able to adapt to the day to demands of working in a busy general practice setting.

Outwood Park Medical Centre is a busy, forward thinking practice and is part of the Brigantes Primary Care Network

We are a friendly supportive team made up of five GP Partners, three Salaried GP's, three Advanced Nurse practitioners, three Practice Nurses, one Healthcare assistant in addition to Secretarial, Admin and reception teams.

We also have a range of additional roles supporting the Practice including Pharmacists, Pharmacy Technicians, First Contact Practitioners, Care Navigators, Physicians Associates and a Paramedic

Professional:

• Offer a holistic approach to travel health by providing comprehensive advice to patients prior to travel including; vaccinations and medicines, safe sex, food hygiene, sun protection.

• Offer advice about childhood and adult vaccinations and ensure vaccines are administered under patient group directions.

• Running well person clinics and health promotion programmes by providing a holistic assessment and lifestyle advice on diet, smoking, alcohol intake and exercise. Facilitates change.

• Able to document Consultations according to NMC guidelines

• Ensures awareness of statutory and local clinical protection procedures, including systems of referral. Ability to recognise signs and symptoms of child abuse.

• Perform a holistic assessment of patients attending for ear irrigation.

• Perform a holistic assessment of patients attending for cervical cytology smear tests.

• Perform venepuncture according to local guidelines

• Suture removal.

• Take ECGs

• Able to recognize and manage anaphylaxis according to current UK guidelines.

• Able to perform Cardio-pulmonary resuscitation according to current UK guidelines.

• Assist in the provision of minor surgery.

• Ability to obtain and document informed consent (either verbal or written).

• Ensure infection control guidelines are maintained.

• Ability to monitor and manage maintenance of stock and equipment to include refrigeration, sterilizer and emergency equipment.
