Job summary

We are looking for a Clinical Coder to join our Workflow Optimisation Team at Northgate Medical Practice.

This is an office-based role and you will play a key part in ensuring accurate and timely clinical coding across the practice, supporting the quality of patient records and the wider work of the team.

Main duties of the job

  • Read and interpret clinical correspondence, discharge summaries, and other patient documentation to extract and code relevant clinical information onto the practice system
  • Assign accurate SNOMED codes to diagnoses, procedures, and relevant clinical data
  • Work closely with clinicians to clarify coding queries and ensure records are complete and up to date
  • Support the Workflow Optimisation team in identifying and managing any coding backlogs
  • Contribute to practice audits and QOF/IIF targets where clinical coding is relevant
  • Maintain strict confidentiality and adhere to data protection and information governance requirements at all times

Job responsibilities

The successful candidate will be an accurate, detail-focused clinical coder with experience of medical records and strong IT skills (including EMIS and Docman). They will be organised and able to manage their own workload, while communicating clearly and courteously with clinicians, administrative staff and patients. Calm under pressure and a supportive team player, they will use tact, discretion and initiative when handling sensitive information and contributing to safe, high-quality patient care.

The main responsibilities include:

  • Accurately enter SNOMED CT codes into patient medical records, ensuring the correct and most relevant codes are used.
  • Ensure all coding complies with practice standards, contractual requirements, and practice policies.
  • Work collaboratively with clinical staff to clarify information and resolve coding discrepancies.
  • Maintain patient records in line with confidentiality, GDPR, and data protection requirements.
  • Assist with audits, reporting, and data validation by providing accurate coded information.
  • Support the practice in achieving Quality and Outcomes Framework (QOF) and other performance targets.
  • Contribute to the continuous improvement of data quality across the practice.
  • Provide a clear and effective handover of any outstanding matters to colleagues before starting annual leave or other planned absence.
  • Input data from new patient health questionnaires.
  • Process changes to patients personal details, such as address and contact numbers.
  • Work in accordance with all Practice policies and procedures.

  • GCSE grade A to C in English and Maths

Experience
