Main duties of the job
What you'll do:
- Code medical records accurately using SNOMED CT.
- Work with doctors and admin staff to keep patient information clear and consistent.
- Help us meet NHS requirements and support important services like QOF.
- Spot opportunities to improve our data and record keeping.

Hours: 08:00 16:30, Monday to Friday
Role
Overview
The Clinical Coder will be
responsible for accurately recording, coding, and maintaining clinical data to
support the delivery of high-quality patient care and compliance with practice
standards. This role requires attention to detail, accuracy, and effective
collaboration with both clinical and administrative staff to ensure that
patient records are complete, consistent, and correctly coded.
Key
Responsibilities
- Accurately enter SNOMED CT codes into patient medical records, ensuring the
correct and most relevant codes are used.
- Ensure all coding complies with practice
standards, contractual requirements, and practice policies.
- Work collaboratively with clinical staff to
clarify information and resolve coding discrepancies.
- Maintain patient records in line with
confidentiality, GDPR, and data protection requirements.
- Assist with audits, reporting, and data
validation by providing accurate coded information.
- Support the practice in achieving Quality and Outcomes Framework (QOF)
and other performance targets.
- Contribute to the continuous improvement of
data quality across the practice.
