An exciting opportunity has arisen for a Practice Nurse, preferably with Diabetes experience, to join our friendly, forward thinking, team. The role will cover a full range of practice nursing duties. We are looking to recruit a Practice Nurse over three days.
Main duties of the job
All Chronic disease reviews with an emphasis on diabetes, childhood vaccinations, smears and any other scheduled vaccinations. Annual vaccines eg flu/COVID

We are a rapidly expanding practice with a practice list size of roughly 7300 patients, five GPs, a part time Practice Nurse, full time HCA and a full administration team. Our clinical system is EMIS Web.

Job Summary:
An exciting opportunity has arisen for an Experienced Practice Nurse at our GP Practice in Prescot, Knowsley. The post holder will provide and maintain a high standard of Nursing care for patients as well as providing Nursing assistance to the Doctors and other members of the primary care team, working together to deliver care within the boundaries and scope of their role, focusing upon nursing treatments, supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities. S/he will work collaboratively with the general practice team to meet the needs of patients, practice key performance indicators and targets, e.g., QOF, support the delivery of policy and procedures and provide nurses leadership as required to healthcare assistants and other workforce colleagues.
This contract is permanent, the successful applicant will serve a six months probationary period, with a view to salary review on completion of probation.
The Post holder will be expected to provide typical practice nurse duties, such as Chronic Disease Management, Cytology, Childhood imms, etc.
