Job summary
The post holder will -
Be
a pharmacist, who acts within their professional boundaries, supporting and
working alongside a team of pharmacists in general practice.
The post holder will work as part of
a multi-disciplinary team
Take
responsibility for areas of chronic disease management within the practice and
undertake clinical medication reviews to proactively manage patients with
complex polypharmacy.
To
provide primary support to general practice staff with regards to prescription
and medication queries.
To
support the repeat prescription system, deal with acute prescription requests,
and medicines reconciliation on transfer of care and systems for safer
prescribing, providing expertise in clinical medicines advice while addressing
both public and social care needs of patient in the GP practice (s).
Provide
clinical leadership on medicines optimisation and quality improvement and
manage some aspects of the quality and outcomes framework and enhanced
services.
To
ensure that the practice integrates with community and hospital pharmacy to
help utilise skill mix, improve patient outcomes, ensure better access to
healthcare and help manage workload. The role is pivotal to improving the
quality of care and operational efficiencies so requires motivation and passion
to deliver excellent service within general practice.
Main duties of the job
The focus of
the post holders work will develop over time and will include -
- Structured Medication Reviews
- Ageing Well MDTs
- Help with care homes enhanced service
- Medication reviews
- Eclipse support monitoring systems
- Assistance with Prescribing Incentive
Scheme and Investment and Impact fund
- Audits and reporting
There will be scope to enhance your clinical skills
in dealing with specific patient groups

consists of 5 GP practices in Northamptonshire (Moulton, Woodview, Kingsthorpe, Wootton and Denton). Our PCN vision is to
help all of our patients lead the best possible life from the cradle to the

face to-face and remote consultations.
This is a PCN role specifically covering Wootton Medical Centre with a combination of practice based and remote working.

- Min 2 to 4 years post graduate (2 years experience working in general practice or 4 years in other comparable healthcare setting).
- Experience of public health promotion/high risk medicines monitoring/lead on antimicrobial stewardship/safe and effective repeat prescribing.
- Good working knowledge of QOF and contribution to QOF delivery.
- Good working knowledge of and role in Enhanced Services.
- An appreciation of the nature of GPs and general practices.
- Able to carry out Pharmaceutical Needs Assessment.
- Able to conduct clinical medicine reviews, reduce waste through poly-pharmacy and over prescribing, reconcile and optimize medications.
- An appreciation of the nature of primary care prescribing concepts of rational prescribing and strategies for improving prescribing.
Desirable
- In depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
- Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (eg patients).
- Produce timely and informative reports.
- To be familiar with GP IT systems.

Qualifications

- Mandatory registration with General Pharmaceutical Council.
- Membership of the Royal Pharmaceutical Society
- Undergraduate Pharmacy Degree experience.

- Other post graduate studies such as a PGDip.

- Masters level Pharmacy qualification or working towards this (MPharm).
