An exciting opportunity has arisen for a self-motivated individual
to join our Extended Care Team.
This role will be primarily based at our Great Hortonsurgery but will require travel to our other sites.
Main duties of the job
The primary focus
of the role is to provide high quality and efficient care to the local
community -
Providing administrative support to the
practice looking after the following extended services: Musculoskeletal, Level
2 Diabetes, Joint Injection, Minor Surgery, Vasectomy, Neurology, Dermatology.
The successful candidate will be required to work 37.5 hours per week.

Job responsibilities
- Answer patient calls and manage appointment bookings (phone lines open 08:00 16:00).
- Register referrals accurately.
- Process and action NHS mail.
- Monitor and send letters from the post tray.
- Scan and attach paperwork to relevant records.
- Check and process documents in the work to do tray.
- Perform data entry tasks to ensure accurate record-keeping.
- Take minutes during meetings as required.
- Work effectively both independently and as part of a large team.
- Follow established guidelines, identifying and updating them when necessary.
- Attend internal and external meetings as required.
- Collaborate with the Extended Care Team Manager to ensure adequate office coverage.
- Assist, direct, and support patients in accessing appropriate services or healthcare professionals in a professional and efficient manner.
- Maintain effective internal and external communication channels.
- Contribute to the continuous improvement of processes by providing suggestions and feedback.
- Outcoming Service Clinics and completing daily data entry to record accurate information, if required
- Meeting monthly deadline for service data check and liaising with the data specialist
- Create/Complete service tasks including chasing patient investigations
- Completing referral triage outcomes and if required liaising with secondary care providers and other non-NHS providers
- Conduct weekly room and stock checks to maintain adequate resources for the service ensuring orders and placed in a timely manner to prevent service disruption
