Job summary

An exciting opportunity has arisen to work as a Health Care Assistant with Evolve Team. The role is with The Five Parks Primary Care Network. The primary focus of the role is to support the EVOLVE Team deliver healthcare in primary care and the community. As an EVOLVE Team member you will help support and improve service delivery for our patients.

Main duties of the job

The primary focus of the role is towork with the current Evolve Team to offer a service across the PCN. This involves proactive and reactive care to patients within the own home, including care homes.

The successful candidate will be required to work 30 to 37.5 hours per week.

Job description

Job responsibilities

The post holder will work with the current Evolve Team to offer a service across the PCN. This involves proactive and reactive care to patients within the own home, including care homes.

The post holder will implement care within the limits of the procedures in which they have achieved competence.

Support and assist other team members to deliver care as delegated and directed.

Demonstrates developed physical skills obtained through practice and training eg accuracy, dexterity and coordination to complete routine tests within agreed pathways of care.

Observes and evaluates progress, conditions or levels and reports to the EVOLVE Team ensuring accurate and timely record keeping is achieved.

Able to identify risk or changes in condition and are responsible for taking action and alerting the appropriate EVOLVE Team member.

Exchanges factual information with patients using persuasion, reassurance, tact and empathy and addressing barriers to understanding to engage the individual in their care and gain valid consent, ensure that all discussions are recorded in an accurate and timely manner in the patient record.

Approaches each individual with care, compassion and sensitivity ensuring that these values are reflected in all aspects of the role, including responding to compliments and complaints.

Ensures that a professional service and image is maintained at all times.

Demonstrates resilience when responding to challenge, change, complex or difficult situations.

To comply with the organisations Infection Prevention and Control requirements including bare below the elbow dress code for staff with a direct care delivery role.

Takes responsibility for their own and others health and safety in the working environment.

Undertakes any training required to develop and maintain their proficiency and response to changes in service delivery or new and emerging techniques within the clinical area.

Reflects on and evaluates own practice and identifies areas of development by setting appropriate objectives.

Contributes to research and development programmes appropriate to clinical practice.

Contributes to sharing good practice and promoting the organisation through presentations and events.

Works in partnership with other agencies and disciplines to effectively support the management of complex cases whilst focusing on individual patient need.

Actively develops and maintains effective clinical and corporate working relationships both within and outside the PCN.

Actively contributes to the multi-disciplinary team supporting the patient, acting as a credible source of information for other agencies / professionals involved in the care as required.

Acts as an advocate for patients and their families / carers, liaising and update members of the EVOLVE Team.

Contributes to the Quality and Outcomes Framework and best practice standards in order to deliver an effective, high-quality service.

Identifies and minimises risks. This includes understanding and applying knowledge in relation to safeguarding.

The post holder will be required to undertake any other duties according to the needs of the service.
