Job summary

An exciting opportunity has arisen for a self-motivated individual to join our Care Navigation team. This role will be based at all our practice establishments - Great Horton, Wibsey, Buttershaw and Horton Bank Top.

Main duties of the job

The primary focus of the role is to receive, assist, and direct patients (care navigation) in accessing the appropriate service or healthcare professional in a courteous, efficient, and effective way.

Provide general assistance to the practice team and project a positive and friendly image to patients and other visitors, either in person or via the telephone.

This is a part time role requiring 25 hours per week, Monday to Friday between 08:00 - 22:00.

Job description

Job responsibilities

To have a thorough knowledge of all Practice procedures.

To work in accordance with written protocols.

Ensure an effective and efficient reception service is provided to patients and any other visitors to the Practice.

Deal with all general enquiries, explain procedures and make new and follow-up appointments.

Explain Practice arrangements and formal requirements to new patients and those seeking temporary cover and ensure procedures are completed.

Receive and make telephone calls as required and take messages, ensuring accuracy of detail and prompt appropriate delivery.

Enter requests for home visits onto clinical system, ensuring careful recording of all relevant details and where necessary refer to Duty Doctor.

Action repeat prescription requests and ensure that they are ready for collection by the patient within 48 hours, either on the computer or manually.

Ensure that all new patients are registered onto the computer system promptly and accurately, including Temporary Residents.

Have an understanding of the services we offer and direct patients to the right clinician.

Chaperone Doctors/Clinical staff where necessary.

Complete all mandatory training when required.

Have working knowledge of telephone system, during and after hours.

Carry out any reasonable administrative tasks requested by any member of the practice team.

Undertake any other additional duties appropriate to the post as requested by the Partners or Managers.

Assist in showing / teaching other staff members and professionals aspects of the job/working day.

Provide holiday/sickness cover for other members of the Care Navigation team.

Ensure building security have thorough knowledge of doors/windows/alarm.

o Open up premises at the start of the day when first to arrive and make all necessary preparations to receive patients.

o When last to leave at the end of the day, ensure that the building is totally secured, and rooms are tidied and ready for the next day.
