Job summary

An exciting opportunity has arisen to work as a PCN Community Matron in a collaborative role. The role is with The Five Parks Primary Care Network.

The primary focus of the role is to help innovate the way we deliver healthcare in primary care and the community, taking a lead role in transforming and improving service delivery for our patients.

We are a dynamic, motivated, and enthusiastic PCN who have successfully employed, pharmacists, social prescribers, mental health practitioners, child safeguarding leads and have paramedics on rotation from Yorkshire Ambulance Service. The PCN covers a large diverse population of patients in Bradford South.

Bradford South is a large PCN looking after 62,000 patients In Bradford. The PCN is made up of 4 practices with a shared vision to deliver joined up effective primary care that has a meaningful impact on the health of all people in our communities, with a focus on reducing health inequalities. We collaborate as a team and work at scale to best meet the needs of our patients, nurture our staff and strengthen the care given by our individual practices.

Main duties of the job

The post holder will work with the current Community Matron to offer a service across the PCN. This involves proactive and reactive care to patients within their own home; including care homes.

As an accountable and autonomous practitioner utilising advanced clinical skills to assess, plan and evaluate care using an expert level knowledge and clinical skills to deliver holistic care, within the candidates sphere of competence and knowledge. This includes referrals and prescribing.

Offer clinical support and supervision to colleagues / students and other clinical staff within the PCN.

Work to professional and regulatory body / codes / standards and guidance at all times ensuring practice is grounded in evidence based theoretical and practical knowledge.

Be involved in developing new ways of working and embrace change.

Managing complex safeguarding cases.

Whilst ACP skills are desirable we will consider applicants from other registered professionals with relevant experience for helping to provide the appropriate care for this cohort of patients with the skill and experience in identifying and coordinating the care that they need. About us

Within the PCN there are a cohort of frail and vulnerable patient including housebound patients, learning disability patients, palliative patients and those with complex health needs. They are often some of the most complex, sickest, and frailest of patients. Traditionally the housebound patients are receiving a home visit at the end of a morning GP surgery list. Due to the pressures we face every day this visit will often only address the most immediate acute needs of the patient rather than understanding some of the more root causes and issues the patients have. Many of these patients struggle to access services at the GP practice or find the issues of trying to get an appointment too much and simply suffer in silence.

Therefore, we are looking to establish and develop the role of the PCN Community Matron that can support and guide our PCN team and help to provide our housebound, frail and vulnerable population with the care and compassion that they deserve. They will form the core of an ageing well team that has GP oversight, is led by the PCN community matrons with access to PCN pharmacy support, PCN paramedics the PCN social prescriber team, the PCN mental health resource, the PCN FCP team, strong links to the DN team and other practice/PCN resources as appropriate.

Utilising advanced clinical skills be able to assess, diagnose and manage acute, sub-acute and chronic conditions, integrating both drug and non-drug based treatment methods.

Ensure clinical practice is underpinned by relevant broad-based knowledge, experience and competence.

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations.

Manage associated admin from visits including referrals, telephone calls.

Prescribe safely, in accordance with local and national guidance and within professional scope of practice. Work with patients, carers and families in order to support adherence to prescribed treatments.
