Job responsibilities

Utilising advanced clinical skills be able to assess, diagnose and manage acute, sub-acute and chronic conditions, integrating both drug and non-drug based treatment methods.

Ensure clinical practice is underpinned by relevant broad-based knowledge, experience and competence.

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations.

Manage associated admin from visits including referrals, telephone calls.

Prescribe safely, in accordance with local and national guidance and within professional scope of practice. Work with patients, carers and families in order to support adherence to prescribed treatments.

Develops and maintains new skills in response to emerging knowledge and techniques.

Where appropriate to service delivery, develop skills which require specific training or considerable experience to attain the level required eg clinical procedures, diagnostic tests.

Demonstrates independent practice responding to social, clinical and ethical issues which are encountered.

Makes complex judgements requiring analysis, interpretation and comparison of options by integrating complex knowledge sources, in new and / or unfamiliar contexts and determining the appropriate application of clinical guidelines relevant to patients needs.

Involved in developing new and innovative clinical pathways across the broad and health social care context.

Engages and actively involves the individual and if appropriate, their family / carer, in the assessment, planning, implementation and evaluation of care / treatment, utilising patient-centred techniques, promote a self-care culture of involvement and empowerment.

Works to standards of advanced proficiency identified by their professional and regulatory bodies performing clinical treatments / procedures to a highly specialist standard.

Prioritises own workload within agreed objectives, appropriately referring to services.

Provides and receives highly complex, highly sensitive or highly contentious information where motivational, persuasive, empathetic, negotiating and reassurance skills are required whilst demonstrating an understanding of barriers to communication.

Approaches each individual with care, compassion and sensitivity ensuring that these values are reflected in all aspects of the role, including responding to compliments and complaints.

Acts as a highly specialist source of information and support for other staff within the PCN and across the broad health and social care spectrum.

To comply with the organisations Infection Prevention and Control requirements, including bare below the elbow dress code for staff with a direct care delivery role.

Proactive case finding, including advanced care planning and frailty reviews.

Undertakes any training required to develop and maintain their proficiency and response to changes in service delivery or new and emerging techniques within the clinical area and demonstrates competence within professional body requirements.

Reflects on and evaluates own practice and identifies areas of development by setting appropriate objectives.

Contributes to research and development programmes appropriate to clinical practice.

Shares good practice and promotes the organisation through presentation, publication and submission for relevant awards.

Participates in devising and delivering training and development to teams / services.

Provides high quality advise, to clinical staff, agencies and individuals.

Works in partnership with other agencies and disciplines to effectively manage complex cases whilst focusing on individual patient need.

Actively develops and maintains effective clinical and corporate working relationships both within and outside the PCN.

Explores the potential for collaborative working and takes opportunities to initiate and sustain such relationships in order to enhance service delivery and make improvements to patient pathways.

Actively contribute to the multi-disciplinary team supporting the patient, acting as a credible source of information for other agencies / professionals involved in the care as required.

Acts as an advocate for patients and their families / carers recognising the boundaries of their clinical knowledge, liaising and referring on to other services / agencies as required.

Contributes to the Quality and Outcomes framework and best practice standards in order to deliver an effective, high-quality service.

Contributes to the development and implementation of services within the PCN, including clinical audit, development of guidelines / action plans.

Identifies and minimises risks. This includes understanding and applying knowledge of clinical role in safeguarding. Reporting, escalating, and managing safeguarding concerns.

The post holder will be required to undertake any other duties according to the needs of the service.

Qualifications

Essential

• Hold a Nursing Qualification and NMC current registration.
• Degree Level / Postgraduate level advanced practice qualification.
• Independent Prescriber / Recognised Non-medical prescribing qualification.

Desirable

• Further professional development.
• MSC / studied at masters level.

Experience

Essential

• Recent experience of face-to-face patient care.
• Understanding and experience in safeguard reporting.
• Sound understanding of legal / professional implications of independent practice.
• Sound understanding of local primary care services.

Desirable

• Experience of managing sub-acutely / acute ill patients at advanced practice level.
• Experience of arranging clinical diagnostics.
• Managing complex safeguarding cases.
• Experience with escalation of patients and arranging hospital.
• Familiarity with computers and SystmOne
