About the Role
The Care Coordinator will play an essential part in supporting our clinical team by helping patients navigate their care, coordinating health reviews, and ensuring key patient groups are proactively supported. You will be the first point of contact for several patient pathways and will work closely with GPs, nurses, administrative staff, and external healthcare providers.
Main duties of the job
Your daily work will focus on supporting vulnerable and high-priority patient groups. Duties will include (but are not limited to):
- Coordinating Learning Disability (LD) reviews and ensuring reasonable adjustments are documented and communicated.
- Managing recalls for Depression Reviews and Dementia Reviews, including liaising with relevant clinicians
- Monitoring patients on NOAC/DOAC therapy,ensuring up-to-date weight records and reviewing compliance.
- Overseeing certain injection therapy schedules, ensuring timely appointments and monitoring safety requirements
- Contacting Bowel Screening non-responders
- Supporting delivery of NHS Health Checks, including invitations, scheduling, and documentation.
- Reviewing Childhood Immunisation missed appointments and contacting families to rebook.
- Conducting and coordinating Cancer Care Reviews, ensuring timely follow-up and holistic support for patients and their families.
- Handling Carer forms, maintaining an accurate register, and supporting carers with available resources.
- Updating the practice website when required
- Assisting with Research projects, including patient ID, invitations, and data collection where appropriate.
- Acting as the main point of contact for local residential home coordinating care and supporting communication between the surgery, carers, and residents.
- Working collaboratively across the practice and with external services to ensure seamless patient care.
