Job summary

We are seeking an experienced and enthusiastic Nurse Practitioner to join our friendly and supportive team at a busy, patient-focused GP practice located in a semi-rural area. As a valued member of our clinical team and the wider Ascent Primary Care Network (PCN), you will play a key role in delivering high-quality, holistic care to our diverse patient population. This is a part-time position (20 hours per week), offering flexibility and the opportunity to make a meaningful impact within a close-knit community setting.

Main duties of the job

The role involves identification and management of minor illness and will require a Minor Illness trained Nurse Practitioner.

It is not a requirement of the role to be a prescriber but that would be a useful qualification. Support for medications will be given to non prescribers by GPs.

Job responsibilities

Main Responsibilities:

  • Provide advanced clinical assessments, diagnoses, and treatment plans for patients presenting with acute conditions.
  • Undertake autonomous clinical consultations, including prescribing following policy, patient group directives and local pathways and referring as appropriate.
  • Support the management of patients with chronic diseases in line with QOF and local guidelines.
  • Contribute to clinical audits, quality improvement projects, and multidisciplinary team meetings.
  • Work collaboratively with the wider practice and PCN team including GPs, practice nurses, receptionists, pharmacists, and social prescribers.
  • Deliver person-centred care and health promotion, tailored to individual patient needs.
  • Ensure that personal and professional clinical standards are maintained.
  • Ensure clinical practice is safe and effective and remains within boundaries of competence, and to acknowledge limitations. Ensure evidenced-based care is delivered at the highest standards ensuring delivery of high quality patient care.

Working Hours:

  • 20 hours per week, with one early start and one later finish to support Enhanced Access.

Qualifications

Essential

  • Registered General Nurse (Registered with the Nursing & Midwifery Council)

  • Recent and relevant experience of working in a minor illness management setting.
