Job summary
We are delighted to offer the opportunity to improve the
employment experiences for a newly qualified General Practitioners, within the
first two years of their GMC speciality registration, by opening a role to join
our warm and patient-centred Primary Care Network. We are looking for GPs to
rotate round our 3 Network practices providing additional access on Saturdays
for routine and long term condition reviews, primarily targeting our working
age populations who struggle to get to appointments in the week. The clinics
are comprised of results reviews, long term conditions reviews and routine face
to face GP appointments. Appointments are 15minutes. There are no acute on the
day appointments or home visits to be done on Saturdays. There occasionally
will be planned routine ward rounds at our local care home. There will
occasionally be a requirement for remote telephone clinics.

Main duties of the job

The GP will undertake a variety of duties governed by the GMS contract and objectives of Enhanced services, including:
- In person consultations
- Telephone consultations
- Visiting patients routinely in their care homes
- Conducting holistic long term condition reviews and managing the patients with evidence based best practice
- Checking and signing repeat prescriptions and medication monitoring
- Dealing with queries, paperwork and correspondence as required
- Clinical administration including processing of results and coordinating referrals for specialist review
- Working autonomously in providing routine pre-booked assessments, diagnosis, and treatment plans
- Providing advice and support for the MDT members on site during your shift.

This post requires the post holder to undertake all statutory and mandatory training and they must be able to undertake physical clinics at the practices.Hours of work will be 9am 5.30pm on Saturdays, as well as possible opportunities to provide evening clinics. 30000 patients across 4 sites (Asplands, Woburn, Fishermead, Walnut Tree), varying from rural to urban locals, affluent to deprived
populations, thus offering a unique opportunity to experience the full width of

We are training practices
with an active frailty support team and a variety of ARRs specialties,
including as First Contact Physiotherapy, Mental Health Practitioners, Social
Prescribing and Health Coaching. We offer a wide range of enhanced services
across the practices including minor surgery, womens health and menopause
clinics, coil/implant fitting, as well as Dermatology and ENT clinics.As we are a PCN with established training links and are highly experienced at supporting and developing clinicians from across the MDT.The PCN has a pro-active focus on identifying frailty and providing early support, improving patient safety through enhanced medication monitoring systems (Abtrace), preventative lifestyle medicine with our wellbeing team, as well as being involved with training university students and GP VTS registrars.

You will be providing reliable access to pre-booked Saturday health clinics, running 9am - 5.30pm, to support capacity at the PCN practices and enhance access to GP appointments for our working age population. Appointments are 15minutes as standard and will be a mix of routine pre-booked GP consultations and planned long term condition reviews. Administration time for processing patient results and correspondence is built in to your day.

Make professional, autonomous decisions in relation to
presenting problems, whether self-referred or referred from other health care
workers within the organisation
Screen patients for disease risk factors and early signs of
illness
Cover the clinical management of the patients in surgery
as appropriate including initiating investigations, reviewing results, making
referrals to secondary care or to other providers as appropriate
Develop care and treatment plans in consultation with
patients and in line with current Practice disease management protocols as well
wider guidance such as that from NICE
Ensure appropriate follow up of patients
Record clear and contemporaneous computerised consultation
notes to agreed standards
Participate in clinical audits, and learning event analyses
and practice meetings
Compile and issue computer-generated acute and repeat
prescriptions avoiding hand-written prescriptions.
Prescribe generically whenever appropriately and in
accordance with the Practice prescribing formulary, NICE guidance and Good
Prescribing Practice (as defined by British National Formulary guidance)
Work with and provide support for other members of the
primary health care team
Play a role in the practice achieving key performance
targets and safety metrics
