Job summary
We are looking to recruit a warm, caring and friendly Patient Navigator to join our dedicated team.
As a Patient Navigator, you will be the first point of contact for our patients, helping them access the right care at the right time. Youll support patients in navigating the healthcare system, signposting them to the most appropriate clinician or community service, and ensuring their experience with the practice is smooth and positive.
Youll be joining a supportive and hardworking team within a fun and friendly working environment. We recognise that theres always a lot to learn in a new role, so we provide a structured induction plan along with ongoing training and support to help you develop your skills and confidence.
Main duties of the job
Frontline support & triage

Serve as the first contact for patients via phone, in person, or online.

Assess and prioritise patient needs to ensure they are directed to the appropriate service or clinician.

Use clinical systems (e.g. EMIS) to book, amend, and manage appointments.

Care navigation & signposting

Guide patients to the most appropriate member of the practice team or external services such as pharmacists, opticians, social prescribers, or community health teams.

Educate patients on how to access services effectively, including online services and self-care options.

Support vulnerable patients (e.g. elderly, those with complex needs) with care coordination and follow-up.

Administrative support

Handle incoming queries, correspondence, and documentation with professionalism and confidentiality.

Maintain accurate patient records and data entry on clinical systems.

Support recall and follow-up processes (e.g. vaccinations, chronic disease reviews, screening programmes).

Communication & teamwork

Work closely with GPs, nurses, healthcare assistants, and administrative staff to ensure smooth patient journeys.

Participate in team meetings and contribute to service improvement initiatives.

Uphold a patient-centred approach and promote equality, diversity, and inclusion.

Undertake any other additional duties appropriate to the post as requested by The Management Team.
