This is an exciting opportunity for a newly qualified Pharmacist to join our well-established Cheltenham pharmacy team as a Foundation Pharmacist. Our team includes 8 Pharmacists (including a Lead Pharmacist) and 3 Technicians. The Foundation Pharmacist is a training role that will lead to undertaking the Independent Prescribing qualification. You will be supporting the delivery of a high quality pharmacy service to meet patient needs and public health and NHS priorities; this is through medicines optimisation and guidance on high quality, safe and cost-effective medicines support as well as supporting people to manage their own health, medicines and long term conditions. This role is aimed at supporting the development and delivery of the future of General Practice healthcare, and advancing the pharmacy profession. Main duties of the job

The post holder will undertake the CPPE Primary Care Pharmacy Education Pathway (PCPEP), followed by the Independent Prescribing qualification that will develop patient consultations and clinical skills. Training will support your work within selected practice(s) in Cheltenham Central PCN, to deliver medicines optimisation that provide high quality, safe, cost-effective and patient centred medication reviews and prescribing. You will work closely with individual practice leads and the pharmacy team to tailor the delivery of network priorities and targets with a practice-centred focus (this list is not exhaustive: see further details in the attachment to this advert).

Key skills/qualifications vital for this role: GPhC registered, sound therapeutic and clinical knowledge & understanding of the principles of evidence-based healthcare, team player and also able to work independently and recognise limitations, excellent verbal and written communication skills, organisational and prioritisation skills, flexibility and adaptability to work pressures.

About us

Cheltenham Central is a mature network of 6 GP practices working closely together. We are led by proactive and forward-thinking Clinical Director who recognises the benefit the pharmacy team can have to General Practice. GPs within our practices have worked to grow and develop the pharmacy team since it was established in 2017. We meet regularly and work together to ensure we learn from each other and can modify and coordinate processes so these are similar between surgeries, working according to confidence and competence of the individual. This ensures pharmacists and technicians can easily swap between surgeries both physically and virtually, should the need arise.

  • To undertake face to face and telephone-based patient consultations, reviewing patients with potential medication issues and long term conditions. This will include proposing de-prescribing within the context of polypharmacy and frailty.
  • Successful candidates will be expected to join the NHSE Primary Care Pharmacy Education Pathway (PCPEP) hosted by CPPE. This is an 18month course, followed by 6 months additional training to qualify as an Independent Prescriber. At the time of this advert, candidates can start the PCPEP pathway as soon as possible after appointment. Candidates can enrol on to the IP once they have been qualified for a minimum 2 years.

Responsibilities underpinning the role

To be enrolled in an accredited training pathway that equips the post holder to be able to practise safely and effectively in a primary care setting and in order to deliver the key responsibilities of the role. Currently, the Clinical Pharmacist training pathways are: Primary Care Pharmacy Education pathway.

To receive support and any necessary supervision from the Senior Clinical Pharmacist, nominated Clinical Pharmacist Mentor and GP lead to allow the post holder to develop and do the job safely and confidently.

To plan and organise the post holders own workload as appropriate including directed audit and project work, and any training sessions for members of the practice team or wider community.

To attend local, regional and national meetings of relevance some of which will be defined within the national pathways for this role supported by CPPE.

To develop and facilitate a good working relationship with community pharmacists and other local providers of healthcare.

To become familiar with System1 GP system and supporting IT and to record personally generated information and maintain a database of information relating to the work done in the practice(s).

Personally generated information includes information and records relating to audit and clinical work undertaken by the post holder, reference notes relating to clinical/technical information, etc.

The database includes maintaining up-to-date, detailed records of all work done in the practices for which the post holder is accountable (done by the post holder or others) using appropriate coding.

To undertake any other duties commensurate with the post holders grade as agreed with the Network/ practice lead GPs or the post holders line manager.

Once registered, to maintain registration as a pharmacist with GPhC and comply with appropriate professional codes and standards.

As appropriate to the post, to maintain and develop professional competence and expertise; keep up to date with medical/ therapeutic evidence and opinion, and local and national service, legislation and policy developments; agree objectives and a personal development plan (PDP) and participate in the appraisal process.
