Job summary

Cheltenham Central PCN is pleased to invite applications from suitably qualified applicants for the position of Care Coordinator. The Care Coordinator will join a team of 1 Care Coordinator and 1 Health and Wellbeing Coordinator to support delivery of excellent anticipatory care. They will identify and manage a caseload of identified patients, making sure that appropriate support is made available to them and their carers. This is achieved by bringing together all the information about a persons identified care and support needs and exploring options to meet these within a single personalised care and support plan, based on what matters to the person. Care coordinators will set up and co-ordinated MDT meetings and be the point of contact for the team.

You will have a shared office based in our new purpose-built health centre, which hosts 3 out of 6 PCN Practices.

Main duties of the job

The Care Coordinator team will ultimately comprise of 2 full time Care Coordinators and 1 experienced Health and Wellbeing Coach, whom will work jointly with the six Practices to support delivery of anticipatory and end of life care.

You will be working with our clinical and non-clinical GP practice teams to support the delivery of the best healthcare to our patients. More specifically, the Care Coordinator will be responsible for helping to plan and coordinate the care needs of patients identified as nearing the end of life within the next 12 months. By creating a joined up care effort with the patient, their families and our local care systems, you'll help us to provide holistic, patient centred care in line with the NHS' Long Term Plan.

Cheltenham Central PCN is highly proactive in developing staff with an innovative approach to Primary Care, and are proud to pioneer new ways to benefit our patients and colleagues across our six member Practices. We are the first PCN to employ dedicated Children and Young People Social Prescribers who work exclusively with 5-18 year olds, and are using a PHM approach to improve health and wellbeing of children and young people in our population. We strongly believe in the importance of staff retention and therefore invest in regular staff support and supervision.
