Job summary
You will be employed as a pharmacist who acts within their professional boundaries, working alongside a team of pharmacists in general practice and supported by a senior clinical pharmacist and GP who will support your development and act as a mentor where required. You will take responsibility for areas of work as identified by the PCN and will need to provide primary support to general practice staff with regards to prescription and medication queries. The role holder will also help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing providing expertise in clinical medicines advice while addressing both public and social care needs of patients in the GP practices.
The role holder will provide clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services. You will also take part in Quality Improvement work and other PCN projects. In the role, you will ensure that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.

Conexus Healthcare is a community interest company and is the federation for general practice in Wakefield.
Wakefield Health Alliance South Primary Care Network (PCN) comprises five GP practices located in the South of the district: White Rose surgery, The Grange Medical practice, Park Green surgery, Langthwaite Surgery and College Lane surgery. Collectively we deliver health care services to approximately 65,000 registered patients.
WHA South is a forward thinking and supportive PCN that encourages personal development and growth. We are committed to improving patient health care with a particular focus on inequalities across the Network footprint.
The PCN team currently consists of GPs, Nurses, Pharmacists, Technicians, Care Co-ordinators, Physiotherapists, HWCs and MHPs. Together we encourage an MDT approach to patient care, integrating the vision of neighbourhood health. We already employ a very successful Pharmacy team and are looking to expand further with an additional Pharmacist who will be supported in delivering the same excellent care and support for both our patients and staff within our busy surgeries.
The role holder will be required to work at any of the five member practices within the South Network.
