Job summary

Role Summary:

Provide primary medical care, diagnosis, and treatment to patients of all ages within your scope of practice as outlined within your individual Boundaries of Clinical Practice Statement (BCPS)

Co-ordinate and manage ongoing healthcare needs, including referrals to specialists

Conduct follow-ups and reviews

Prescribe medication (for Independent Prescribing Pharmacists only and within their scope of practice)

Monitor and manage chronic and acute conditions

Offer health promotion and preventative care advice, including lifestyle modifications and disease prevention strategies

Work as part of the practice multidisciplinary team, delivering care to the registered patient population

Reports into a named Lead Pharmacist

Role Summary:

Role Responsibilities:

Provide primary medical care to registered patients, including assessing, diagnosing, and treating a wide range of conditions including chronic diseases, minor illness and minor injuries within your scope of practice as outlined within your individual Boundaries of Clinical Practice Statement (BCPS)

Consult with patients utilising clinical pharmacological expertise to listen, diagnose, and develop appropriate treatment plans

Interpret test results and recommend referrals to specialists or healthcare professionals when necessary

Prescribe medications appropriately (Independent Prescribing Pharmacists ONLY)

Manage acute and chronic illnesses, with a focus on preventive medicine and health promotion, delivering opportunistic health promotion

Maintain accurate and up-to-date patient records, ensuring confidentiality and adherence to GDPR guidelines

Collaborate effectively within a multidisciplinary team to ensure seamless care

Support and contribute towards practice clinical meetings, peer reviews and other educational forums to enhance knowledge and skills

Maintain a log of CPD (continuous professional development) including attainment of elements of pre-agreed PDP (personal development plan)

Stay up to date with training and educationincluding medical advancements and new guidelines / regulatory changes relevant to delivering primary care services

Adhere to clinical governance and adherence to professional codes and standards

Show empathy, professionalism, and sensitivity whilst communicating with patients, to ensure a patient-centered approach that respects cultural and individual needs

Log and escalate any concerns regarding newly identified SEAs

Participate in local initiatives to enhance service delivery and patient care

Carry out audits and support the practice in management practices such as QOF and compliance

Qualifications

Essential

  • MPharm or equivalent
  • Full and current registration with the General Pharmaceutical Council (GPhC) and included on the GPhC Register
  • Completion of a 1-year training program in Pharmacy (Pre-registration)
  • Successful completion of the GPhC pre-registration examination

Desirable

  • Membership of Royal Pharmaceutical Society (RPS) or equivalent
