Bank Medications Management Assistant

Job summary

To liaise with all members of the clinical team, patients, and other healthcare professionals to provide an accurate, timely and qualitative process for prescription issues and medication updates for patients registered at Montpelier Centre.The post holder will be expected to deal with queries from patients, local pharmacies, hospital departments, nursing homes and GPs and the Clinical Pharmacist daily.

Main duties of the job

Liaise with all members of the practice team, and in particular with the clinical staff to ensure that patients prescription requests are responded to within the agreed timescales.

Liaise with the relevant pharmacies where there are queries regarding a patients prescription.

Liaise with other members of the medicines management team at times when additional support is needed to deal with medication changes/dossettes/documents to agree workload delegation. If necessary, this can be discussed with line manager or PM.

In conjunction with the line manager/PM ensure that all prescription coordinator duties are covered during absence for any reason.

Monitor changes to medication of any over and under use and bring to the attention of the regular Clinician.

Use judgement of whether medication should not be issued on a repeat.

To generate patient requests for prescription items not listed by liaising with the relevant clinical team member

Deal with all discharge summaries and any other clinical documents relating to changes/introductions of new medications to the medication changes inbox within the EMIS workflow (once coded) on the days assigned to do documents.

Update the changes on patients medication screen within EMIS.

Forward documents on to the GP where necessary.

Responsibility for compilation and update of dossett boxes.
