Job summary
Parkfield Medical Centre has an opportunity for a Clinical Pharmacist to join our team on a fixed term contract covering maternity leave. The successful candidate will be responsible for providing high quality pharmaceutical care to the patient population. The pharmacist will work as part of a multidisciplinary team and will ne responsible for assessing, diagnosing, prescribing and managing medications within their competency to ensure safe, cost-effective prescribing.
Main duties of the job
Medicine reconciliation following discharge from hospital
Review of High Risk Medication
Blood test monitoring and therapeutic drug levels
Authorisation of repeat prescriptions
Medication queries from patients and outside agencies
Cost effective prescribing in line with local guidelines
Audit to improve prescribing safety
Development of practice policies and protocols
Monitoring and management of CQC searches
Processing of CAS alerts
Advise patients on medication use
Provide lifestyle and self-care advice where appropriate
Support care homes with medication reviews
Point of contact for medicines advice to the wider practice team
Reduction in medication errors.
Provision of SMRs
Long term condition management
Deprescribing and polypharmacy reviews
