Job summary

We are seeking enthusiastic and reliable individuals to join our team as part-time Chronic Disease Nurse. You will be working as a team of practice nurses. This role primarily involves managing patients on chronic disease registers.The ideal candidate / extended skills in Chronic Disease Management in Diabetes, COPD, Asthma, CHD.

This role offers up to 30 hours per week.

Main duties of the job

All aspects of Chronic Disease Management , including the implementation of processes for the effective management of patients with long-term conditions using evidence-based practice including care for patients.
