We are looking to recruit a
motivated, caring and enthusiastic, Chronic Disease Nurse to join our friendly and
supportive nursing team. We are looking for a highly motivated individual with excellent communication and interpersonal skills.
You will be responsible
for ensuring the delivery of safe and effective nursing care to the whole
practice population, working closely with the wider Practice Team to deliver our practice priorities.
Main duties of the job
Clinically, the focus of the role is management of patients with chronic conditions specifically Diabetes and Asthma.
The practice has surgery premises at Keynsham and Saltford. Your main
base will be Keynsham, but you may be required to work at other surgeries, or
at such other locations in the local PCN as the Practice may reasonably direct.

population, within the boundaries of the role. He/she will support patients to
be healthy, monitor patients with long-term conditions, and deliver health
prevention, promotion and screening activities.
This
will include but is not limited to assessing patients; planning and
implementing treatment protocols; and reviewing and tracking progress.
The
practice nurse will work collaboratively with the practice team, performing tasks
as required by the lead practice nurse and the partners, within the practices
policy and procedures, and to a defined set of competencies.

St Augustine's Medical Practice is based in the new Chocolate Quarter development in Keynsham, we have a small branch surgery in nearby Saltford.
Our purpose built modern spacious premises, easily commutable from Bath/Bristol and surrounding areas and a short walk from Keynsham train station / high street. There are many amenities at the Chocolate Qtr including an on-site gym/spa facility, with pool, offered at a discounted rate for NHS staff, as well as coffee shop and restaurant.
Our friendly nursing team of ANP's, Practice Nurses and HCAs is part of our diverse and changing workforce, which consists of GPs, FCP's , paramedics, pharmacists, health care assistants, and other allied health professionals.
We have regular meetings to manage and continually improve our patient care. We are passionate about training the next generation of workforce having been teaching practice for many years.
