Job summary

We have an exciting new vacancy for a care navigator. You would be the first point of contact for patients, helping to manage their journey through the healthcare system by coordinating services and providing administrative and reception support.

The role is for 25 hours per week.

Key responsibilities include answering patient queries, booking appointments, directing patients to the appropriate healthcare professional or service, and providing administrative support such as filing.   • Patient communication:Serve as the primary point of contact for patients via phone, email, or in-person, managing inquiries and directing them appropriately.
  • Appointment management:Book appointments for patients and manage schedules to optimise the patient journey.
  • Administrative support:Handle tasks such as data entry, filing, and assisting with the long-term conditions recall and DNA management.
  • Service coordination:Signpost patients and their families to other services, including statutory, voluntary, and community support networks, to help manage their needs and improve their quality of life.
  • Liaison:Act as a communication hub between patients, doctors, and other medical staff.

Required skills and qualifications

  • Communication:Excellent verbal and interpersonal communication skills are essential, including a polite and clear telephone manner.
  • Technical proficiency:Ability to use IT systems for administrative and data entry purposes.
  • Problem-solving:Ability to work under pressure and find the best solutions for patients' needs.
