We are seeking a dedicated and compassionate Advanced Nurse Practitioner to join our healthcare team.

The ideal candidate will provide high-quality patient care; demonstrate clinical expertise; have an interest in elderly care; have experience in Palliative & Dementia care and experience in clinical triage.

Duties:

To help us deliver Enhanced Health in Care Homes

Conduct comprehensive physical assessments

Diagnose and treat common acute illnesses and injuries

Manage chronic health conditions

Prescribe medications and treatments

Order and interpret diagnostic tests

Collaborate with physicians and other healthcare professionals

Maintain accurate patient records

Qualifications:

Registered Nurse qualification with valid Nursing and Midwifery Council NMC registration

Completion of an accredited Nurse Practitioner programme

Proven experience as a Nurse Practitioner in a clinical setting

Excellent communication and interpersonal skills

Strong critical thinking and decision-making abilities

Hrs - 9.30am - 3.30pm - Monday - Friday

Main duties of the job

The post holder will:

Have an interest in elderly care to help us deliver the enhanced health in care homes framework into our Residential and Nursing Homes within the boundaries of our General Practice moving away from traditional reactive models of care delivery towards proactive care that is centred on the needs of the individual residents, their families and care home staff.

have at least 1 years Advanced Clinical Practice experience

be responsible for contributing to the ongoing development of clinical practice, research and standards of care within the service, including development of policies and guidelines and contribute to service development to ensure patients have a seamless pathway.

be expected to practice autonomously, formulate clinical decisions and treatment pans within local guidelines/policy to manage both acute and chronic illness.

We would welcome applications from practitioners with experience in the long-term health needs of our aging population.

The post holder will require excellent communication skills which should include communicating clearly, sensitively and effectively with patients, families and professionals. Be able to signpost patients to the most appropriate services and be enthusiastic to apply acquired knowledge and skills.

2 x Practice Nurses

1 x Health Care Assistant

2 x Phlebotomists

Plus a full compliment of Administrative staff

We are part of the Stone & Eccleshall Primary Care Network

PCN Staff include:-

First Contact Practitioner; Urgent Care Practitioner; Team of Pharmacists; Pharmacy Technician; Social Prescriber; Mental Health Worker - all of which work across the 3 Practices within the PCN network.
