Job summary
As Community Health and Wellbeing Worker you will be on the
frontline working with households on the Doddington and Rollo estates. This
role aims to improve the health and wellbeing of the communities they serve.
You will play an important role in promoting health and healthy lifestyles, prevention and early detection, and supporting residents engagement in their health through a range of activities including monthly door knocks, regular outreach events, building a community network brief intervention with residents, community education, signposting and referring on to third sector and professional services, informal counselling, and advocacy.
The family health model which this role implements needs to
be flexible and dynamic so the work also represents an opportunity to continue
to shape an emerging role in our healthcare system.
Main duties of the job
The main duties include carrying out monthly
household visits (or more frequent if required) within a
specific area of the Estate to assess the health and wellbeing needs of
everyone within a household, adopting a proactive and holistic approach when
supporting the local community. The main tasks are:

This role will be working with Battersea Fields Practice, which is one of 5 practices in Battersea
Primary Care Network. Battersea Primary Care Network is located in a compact area of North Battersea and also comprises of Bridge
Lane Group Practice, Lavender Hill Group Practice, Queenstown Road Medical Practice and Battersea Rise Group Practice. The Primary Care Network has a combined patient population of approximately 54,000. We enjoy excellent collaborative

The role comprises of the following duties:
Relationship
Building and Health Promotion
- Build
relationships with assigned households to understand their needs and context;
- Offer
health coaching and motivational approaches including problem solving and goal
setting;
- Deliver
personalised health promotion and support healthy choices and education, such
as breastfeeding and immunisations whilst providing health literacy support;
- Provide
support and signposting with lifestyle advice such as smoking cessation,
alcohol consumption, healthy diet and physical exercise;
- To
support households to understand the health and social care system.
Health

- To support
those eligible for childhood immunisations and adult health and cancer
screening appointments and encourage uptake;
- To
support residents with chronic disease by enabling necessary monitoring and
medication compliance;
- To
encourage early identification of signs and symptoms of chronic illness and
share the information as provided by practices;
- To
identify household determinants of ill health and health seeking behaviour;
- To
signpost and refer people to services across the health and care system and
other existing community services.
Day to Day
Administration
- Keep
digital records that reflect household and community need and progress via
secure tablet;
- To
comply with data protection and confidentiality;
- To maintain
skills in using technology and primary care software;
- Present
your work and findings to the local GP and multidisciplinary team;
- Attend
weekly supervisor meetings with clinician in GP surgery and regular meetings
with the project team.
Community Engagement/Co-Production
- Engage
with the community to ensure health services are satisfactory and appropriate
in their design and delivery;
- Facilitate
networks within communities to build community capacity;
- Managing
conflict;
- Advocate
needs as outlined by the community;
- Identify
local community assets and promote a community-based prevention agenda;
- Together
with the project team, engage with South West London NHS and public health
teams around ongoing evaluation of this new model and in sharing community need;

in supervision and debriefing.
