Main duties of the job

As an Advanced Nurse Practitioner, you will deliver high-quality, patient centred care while working autonomously within your professional scope. You will assess, diagnose, and manage patients, making clinical decisions in line with established procedures and protocols set out by our GPs.

Key responsibilities include:

  • Diagnosing and treating patients independently
  • Prescribing medications safely and appropriately
  • Requesting and interpreting investigations
  • Delivering evidence based interventions to optimise patient outcomes
  • Working collaboratively with our multidisciplinary team to ensure excellent continuity of care

About us

We are based in a leafy, suburban area of North-West Bristol; conveniently close to the M5 motorway and an easy and pleasant drive from central Bristol. We enjoy the benefits of a bright, modern, purpose-built Health Centre.

Our 11,200 patients are looked after by 6 clinical Partners, 6 salaried GPs and an ANP, with support from our experienced Treatment Room, Administration and Patient Services teams.

We are a hard-working and close-knit bunch, with an emphasis on humour and peer support to meet the current challenges in primary care. We enjoy entertaining staff nights out and there is always plenty of chat and information flow on our WhatsApp groups.

Job responsibilities

Clinical

  • Receive patients with undifferentiated and undiagnosed problems and make an assessment of their healthcare needs
  • Diagnose and manage presenting conditions using advanced clinical skills, critical thinking, and evidence based practice.
  • Provide urgent, same day, and routine appointments for patients of all ages.
  • Triage patient queries and manage clinical workload efficiently.
  • Provide health promotion, disease prevention, and lifestyle advice.
  • Assess, manage, and treat minor injuries and illnesses.
  • Safeguard children and vulnerable adults, following local and national policies.
  • Prescribe medications and treatments independently within professional competencies.
  • Assessment, diagnosis, treatment, prescribing, referral and discharge as appropriate and deliver key elements of NSFs, providing public health, health protection and promotion programmes that improve health and reduce inequalities
  • Act as clinical governance lead for the Practice and ensure significant events are reported using Practice procedures
Administrative Responsibilities
  • Maintain accurate, legible, and contemporaneous patient records.
  • Use the digital system EMIS effectively for consultations, referrals, and medication management.
  • Support delivery of QOF, and other contractual requirements.
  • To collect data and participate in regular Practice audit and research projects
  • Contribute to the development of practice policies and service improvements.
Other
  • Work closely with GPs, nurses, pharmacists, social prescribers, and external services to ensure integrated patient care.
  • Liaise with secondary care, community teams, and allied health professionals as needed.
  • Participate in MDT meetings, case discussions, and safeguarding forums.
  • To conduct home visits as appropriate and necessary
  • Support the development of protocols, pathways, and best practice guidelines.
  • To maintain a safe and therapeutic environment for all Primary Health Care Team staff, patients, carers and co-workers from other agencies. Ensure that all statutory regulations e.g. Health & Safety, COSHH, etc are implemented and adhered to.
  • To promote a professional approach at all times by attire and attitude to patients and staff both in and out of the Practice
  • To maintain own continuous professional development and ensure PDP kept up-to-date in line with the requirements of the Practice
