Guardian Medical Centre is seeking an experienced Practice Nurse
to join our friendly, dynamic, and forward-thinking team. Working days and hours are negotiable for the right candidate.
We are
well-established training practice with a list size of 10,850 patients and you will be supported by an experienced multi skilled team of
clinical and non-clinical staff.
Main duties of the job
You will work as part of the multidisciplinary team responsible for
delivering a high standard of nursing care. You will have excellent
clinical skills with experience in managing chronic conditions. Ideally you will be passionate about providing high
quality care to our practice population. You will be a confident and friendly
team player with excellent communication skills, a positive can do attitude and
commitment to general practice.

Job responsibilities
Primary clinical
responsibilities:
- Develop,
implement and embed health promotion and wellbeing programmes
- Provide
chronic disease clinics, delivering patient care as necessary, referring
patients to secondary/specialist care as required
- Implement
and evaluate individual specialised treatment plans for chronic disease
patients
- Manage
patients presenting with a range of acute and chronic medical conditions,
providing subject matter expert advice
- Identify,
manage and support patients at risk of developing long-term conditions,
preventing adverse effects to the patients health
- Maintain
chronic disease registers
- Perform
annual pill checks
- Vaccination
& immunisation programmes for adults and children
- Deliver
opportunistic health promotion where appropriate
- Venepuncture
- Undertake
the collection of pathological specimens
- Provide
travel medicine services
- Request
pathology services as necessary
- Maintain
accurate clinical records in conjunction with existing legislation
- Prioritise
health issues and intervene appropriately
- Support
patients in the use of their prescribed medicines or over the counter
medicines (within own scope of practice), reviewing annually as required
- Contribute
to practice targets (QOF etc.), complying with local and regional guidance
- Chaperone
patients where necessary
- General
nurse clinical duties
- Liaise
with external services/agencies to ensure the patient is supported
appropriately (vulnerable patients etc.)
- Delegate
clinical responsibilities appropriately (ensuring safe practice and the
task is within the scope of practice of the individual)
- Support
the clinical team with all safeguarding matters in accordance with local
and national policies
- Support
the team in dealing with/escalating clinical emergencies

- Process
nurse appropriate clinical mail
- Vaccination
stock checks
- Vaccination
stock ordering
- Monitoring
of vaccination cold chain/fridge temperatures
- Assist
other members of the practice team as appropriate
