Job responsibilities
Operational Management
To work with
and support the PCN Clinical Director to develop and deliver the PCN strategic
plans and performance requirements.
To provide
oversight and accountability to the delivery of PCN contractual requirements
and specifications.
To support
the progress of projects and manage performance to achieve the strategic vision
of the PCN.
To support
the PCN and its member practices to formulate project and delivery plans in relation
to meeting the DES specification and oversee the management of and delivery
against the DES requirements.
To work
closely with PCN practices to develop an understanding of their different
demographics, population health needs and an understanding of health
inequalities.
To be the
first point of contact for Practice Managers and commissioners / ICB in regards
to PCN operations.
Assist in
the coordination of the PCN meetings and other development meetings; including
setting dates, drawing up agendas and communicating decisions to the wider
PCN.
Support in
the writing of business cases and bids on behalf of the PCN.
Implement,
monitor and report on PCN contracts and services, including project progression
and management of risks.
Collaboration
Recognise
the roles of other colleagues within the PCN and across other organisations and
how their role impacts patient care.
To proactively
build relationships with system leaders to help influence proactive and new
models of care.
Liaise with
key partners in the development and monitoring of integrated services and
projects.
Demonstrate
use of appropriate communication to gain the co-operation of relevant
stakeholders including patients, carers, senior and peer colleagues and other
professionals across the ICC and within ICB.
Finances
To support
in the budgeting and monitoring processes to maintain the short and long term
financial management of the PCN.
Prepare and
present financial reports, monitoring information and reporting to the PCN
board as required.
Ensure that
the PCN maximises National and regional funding streams available to support
ongoing and innovation in care delivery provision to the PCN population.
Recognise
any financial risks and take immediate and appropriate action.
Check,
approve and sign off any financial claims.
HR
To provide
line management alongside PCN clinical lead and supported by service support
lead to PCN employed staff, ensure they are embedded within the PCN and
coordinated across practices.
In
collaboration with CD, clinical lead and service support ensure all PCN
employed staff have annual appraisals, clinical and management supervision
appropriate to their role.
Oversee and
be accountable to find resolution for all disputes and grievances related to
PCN employed staff, act as a point of expertise to inform and guide processes.
Data & IT:
To work
closely and assist PCN practices with the implementation and training on new IT
systems to ensure unified consistency over the PCN.
Manage and
direct the collection of data for continuous quality improvement which should
be measurable and auditable.
Ensure data
collected from practices is accurate and relevant.
Work
alongside practices to understand current challenges to effective service
delivery and identify solutions, including promoting role out of existing best
practice within the PCN.
Ensure all
change is focussed on improving patient care and is based on working with
patients and neighbourhood teams to understand their needs and their experiences with robust evaluation.
Use
improved techniques to focus on delivery of changes and analysis of their
impacts, providing feedback to staff whether positive or negative impacts of
changes made.
Assess the
PCNs transformation maturity and help the ICS understand local support needs.
Build
relationships with practices across the PCN to develop a consensus for
improvements and to ensure all staff (clinical and non-clinical) arefully
engaged throughout the process of implementation, deployment and monitoring /
evaluation of change, peer challenge and learning.
Develop and
manage an overall transformation plan for the PCN, including identifying
interdependencies, managing risks, considering the potential impacts on the
wider organisation and determining resource requirements.
Link with
other PCN D&T leads, ICB, ICS and other system partners to collectively and
collaboratively solve problems, share learning and gain buy in for
transformation and to support alignment with wider strategy and improvement
approaches.
Use data to
advise strategic planning to address population health management and ensure
addressing of any health inequalities across the PCN.
Advise on
innovative opportunities and support practices to maximise benefits from
innovation, through the sharing of learning and best practice.
Develop and
implement mechanisms for assessing the impact of agreed changes.
Keep PCN
leadership and member practices updated on progress of current initiatives,
capacity for change, progress and issues so that plans can be adapted in response.
Take a data
driven approach, looking at evidence and information available to support
understanding of challenges, identify possible solutions and track the impacts
of changes that have been made.
Work in
partnership with the ICB ICS to identify relevant data sets and to support
standardisation in the use of measures for understanding change and improvement
in General Practice.
Bring
together data from various sources and use information to develop insights into
working practices, or support population health management, risk stratification
or workforce planning effectively tailoring content to meet he needs of the
audience.
Effectively
present and communicate data to practices and PCN staff at all levels to
increase understanding of operational challenges.
Help PCN
and practice staff understand the limitations of particular data sets where
other information may be available to supplement understanding of issues.
Support and
encourage a focus on data in understanding the effectiveness of changes made.
Seek and
secure analytical resource as required to support the PCN in developing and
implementing appropriate data and information sharing mechanisms.
Confidentiality
In the
performance of the duties outlined in the job description, the post holder may
have access to confidential information relating to patients and their carers,
practice staff and other healthcare workers. They may also have access to
information relating to the member practice as a business organisation and to
the PCN. All such information from any source is to be regarded as strictly
confidential.
Information
relating to patients, carers, colleagues and other healthcare workers or the
business of the member practices or Network may only be divulged to authorised
persons in accordance with the Network policies and procedures relating to
confidentiality and the protection of personal and sensitive data.
In the
course of seeking treatment, patients entrust us with, or allow us to gather
sensitive information in relation to their health and other matters. They do so
in confidence and have the right to expect that staff will respect their
privacy and act appropriately.

The PCN is

holder will be required to support the PCN continually strive to improve work

and staff will be permitted to make suggestions and contributions to improve
our service delivery and enhance patient care.
The post holder
will maintain a good knowledge of key policies from the department of Health
and Social Care, NHS England and other relevant government departments and
agencies and support PCN staff to understand and interpret these locally.
Communication / Collaboration
The
post-holder should recognise the importance of effective communication and
collaboration both within and outside the organisation, acting as an ambassador
for the network will strive to:
Proactively
build relationships with system leaders to help influence proactive and new
models of care
Recognise
peoples needs for alternative methods of communication and respond
accordingly.
Develop
relationships across the PCNs, ICB/ICS and other service delivery
organisations, patients and communities to plan and deliver changes that will
improve working between general practice and other primary, community and wider
care services.
Lead or
participate in relevant working groups, project groups, service meetings and
other forums across the PCN, ICB/ICS to agree changes, share learning, discuss
challenges and resolve issues.
The
post-holder will be expected to carry out other tasks, which may be reasonably
required by the employer.
