Job summary
Kingswood Health Centre are looking for a new Prescription Clerk to join our well-established Pharmacy Team.
The prescription clerk will work with the Senior Prescription Clerk, our Pharmacy Technician and team of Clinical Pharmacists, to help provide a seamless service to patients with regards to medication ordering and prescription queries.
This role is for 30 hours per week, worked Monday to Friday to support the needs of the team.
Main duties of the job
To act as a prescription
clerk at Kingswood Health Centre, dealing with receiving requests from patients
and/or representatives and allied professionals for prescriptions to fulfil
medication requirements. Requests will be channelled via various sources (paper
submissions, email, online). Ensuring patients are up-to-date with reviews and
assisting and directing patients in booking their appropriate appointments to
ensure their care is ongoing and that their health is reviewed at the
appropriate time and with appropriate clinical follow-up.
In addition the clerk role
is to provide general assistance with ensuring patient medication is correct
and current at all times by checking any communications received from hospital
outpatient letters, discharge information and communications and liaising with
a member of the pharmacy where necessary to ensure transcription, addition and
removal of mediations as appropriate is done at the earliest opportunity and
the patient and pharmacy informed using the correct contact format.

Job responsibilities
- Deal with medicines requests from all patients, including vulnerable patients (for example those with 7-day dispensing/patients needing post-dated scripts)
- Liaise with local pharmacies to gain an appreciation of current stock shortages
- Keep the out-of-stock medication board up to date with the support of the Pharmacy Team
- Deal with requests for out-of-stock medication alongside support from the Senior Prescription Clerk and the Clinical Pharmacists
- Deal with patients requests, ensuring that patients are notified of the progress of their request and that they are booked in for reviews where appropriate
- Have an understanding of EPS and electronic repeat dispensing (eRD)
- Be able to track prescription requests through the EPS Prescription Tracker Portal
- Ensure safe, efficient and smooth transfer of prescriptions electronically to the patient's pharmacy of choice, updating records where nominations change
- Complete communication forms and submit to the relevant pharmacies when updating patient medication which has been submitted via eRD
- Support the Pharmacy Team with any searches or medication-related switches which may be required
- Check and amend patient demographics at every contact to ensure records are up-to-date and accurate at all times
- Handling requests for prescriptions which require clinical input from the Pharmacy Team or other clinicians using the appropriate messaging format and keeping the patient updated on their request
- Monitoring of the prescriptions email inbox and Klinik inbox, actioning and responding appropriately
- Action documentation that has been forwarded to the prescription clerks
- Ensure monthly order requests from nursing and residential homes are completed in a timely manner
- Ensure any orders for appliances are in line with the BNSSG ICB Formulary and that the patient is aware that an order has been requested on their behalf if being requested by the appliance contractor directly
- Liaise with patients to gain further information regarding script requests for past/acute medication where needed
