Job summary
Kingswood Health Centre are looking for an additional Clinical Pharmacist to join our well-established Pharmacy Team.
You will work alongside the Pharmacy Team Manager and another Clinical Pharmacist (both independent prescribers), a Pharmacy Technician and a Prescription Clerk.
Our Pharmacy Team are a key part of our practice and provide an outstanding service to our patients. This new post is on offer for between 25 - 30 hours per week. It will predominantly be based at the health centre but there will be some opportunity for remote working, workload depending.
Main duties of the job
To be responsible for
an effective medicine management system within the practice, including
cost-effective prescribing, medicines reconciliation, chronic disease
management and medicines optimisation. Ideally working as an independent prescriber
(although training can be provided), providing advice and support for both
patients and colleagues.

The
following are the core responsibilities of the clinical pharmacist. There may
be on occasion, a requirement to carry out other tasks; this will be dependent
upon factors such as workload and staffing levels:
- Practice as an independent prescriber working within scope of practice, where appropriate/qualified
- Carry out face-to-face and telephone consultations with patients with chronic health conditions and medication queries
- To provide medication review services with patients in the practice and during domiciliary visits to the local nursing homes
- To provide advice and support for patients with queries relating to medications and to carry out health checks and reviews in relation to specific chronic disease areas
- Encourage cost-effective prescribing within the practice
- Review all safety alerts ensuring the required actions are taken
- To reconcile medicines following patients' discharge from hospital or clinic appointments
- Review medications for newly registered patients
- To provide subject matter expertise on medication monitoring, implementing and embedding a system
- Carry out targeted medication reviews for high-risk patients and medication reviews requested by GPs
- Identify patients suitable for repeat prescribing, liaising with the relevant clinicians as required and streamlining the process where appropriate
- Review and implement safe prescribing systems for vulnerable patients, improving compliance
- Support clinicians with the management of patients suffering from drug and alcohol dependencies
- Actively signpost patients to the correct healthcare professional
- Assist with QOF and Prescribing Quality Scheme targets
- Assist with the management of incoming lab results
- Liaising with local pharmacies about medication supply issues
In additional to the primary responsibilities, the clinical pharmacist may be requested to:
- Partake in audits and QOF QI projects as directed by the audit/prescribing lead
- Provide education on medicines for the clinical team
- Review the latest guidance ensuring the Practice conforms to NICE, CQC etc.
- Act as the Practice point of contact for all medicines-related matters, establishing positive working relationships
