Job summary

We are looking for a friendly experienced Practice Nurse who ideally specialises in chronic disease, to join our well-established nursing team.

The successful candidate would be expected to have experience of working as a practice nurse and with Chronic Disease patients and be flexible to cover general aspects of practice nursing.

The expectation being the individual will assist in our management of the case load for chronic disease patients within the guidelines and protocols for the wellbeing of patients on the Chronic Disease registers.

As a general practice nurse the candidate will have the capability to make sound judgements and will be expected to advise patients on general health care and health promotion.

  • Chronic disease management/long term conditions within the clinical setting.
  • Treatment room duties
  • Health promotion
  • Cervical cytology
  • Routine immunisations/childhood immunisations.
  • Treating minor injuries
  • Travel health advice and vaccinations.
  • QOF awareness
  • Assisting GPs with minor surgery and coil fittings
  • Smoking cessation.
  • Requesting pathology tests.

About us

Come and join our dedicated and welcoming team based in Yate, part of South Gloucestershire.

Our practice has 7 partners and a committed team of clinical, non-clinical and allied health professionals providing care to approximately 15,000 patients.

The practice is part of the Yate & Frampton PCN made up of 5 GP practices that work together with the aim of improving healthcare for the community and reducing health inequalities and has good relationships with the PCN, local ICB and PPG.

5 weeks annual leave offered and 8 public holidays (pro rated).

This is a part time position, days required Wednesday and Friday.

Job description

Job responsibilities

Assist in our management of the case load for Chronic Disease patients within the guidelines and protocols for the wellbeing of patients on the Chronic Disease registers.

To oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required

Liaise with the Nurse Manager to provide nursing services to the practice, to allow adequate cover at all times.

Provide nursing treatment to patients in participation with General Practitioners or independently within agreed protocols.

Advise patients on general health care, health promotion, illness prevention, minor ailments, acting as counsellor and adviser as required, with referral to GP as necessary.

Assist in minor operations if called upon.

Advise patients on test results and liaise with GP for prescriptions as necessary.

Provide telephone advice to patients as required.

Undertake diagnostic tests blood and urine tests, ECGs, carry out test and follow ups as necessary. Discuss results with patients as necessary.

Treatment of minor injuries, dressings, removal of sutures, routine injections, immunisations and ear syringing as required.

Undertake the responsibility for infection control within the surgery, eg equipment cleansing and sterility.

Undertake correct collection and storage of pathological specimens including intravenous blood samples, swabs, smears etc.

To implement and evaluate individual treatment plans for chronic disease patients that promote health and well-being

To identify, manage and support patients at risk of developing long term conditions, preventing adverse effects to patients health

To provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF

To oversee the provision of wound care services to patients

To co-ordinate the provision of travel medicine services

To request and process pathology services as necessary

To maintain accurate clinical records in conjunction with extant legislation

To ensure EMIS codes are used effectively by all members of the nursing team

To chaperone patients where necessary

To prioritise health issues and intervene appropriately

To support the team in dealing with clinical emergencies

To recognise, assess and refer patients presenting with mental health needs

To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)

To support the clinical team with all safeguarding matters in accordance with local and national policies
