Duties And Responsibilities
Organisational:
Work to support the PCNs objectives.
Prioritise co-developing mechanisms involving all practices to enable delivery of the PCN DES and high achievement in the Investment and Impact Fund (IIF).
To keep the PCN board informed of project progress and to work with members of the board in each area to deliver successful completion.
Support the PCN with writing business and development plans as required.
To provide up to date data measuring PCN achievement in terms of DES and IIF delivery.
Identify and record risks and issues, developing contingency plans with service and contract leads.
Ensure all relevant PCN national strategies, policies, targets and frameworks are considered and implemented.
To manage the recruitment, induction and training of new and existing PCN staff contracted via ARRS.
To provide management and support to PCN employed staff and to manage their operational rotas and leave requests.
Undertake day-to-day line management for rota changes and staff absences for example, leave and sickness.
Management of complaints.
Appraisal of PCN staff as appropriate
Maintain clinical data reporting in line with PCN deliverables, support and work with practices which may be experiencing difficulties achieving PCN targets.
Financial management of the PCN, including budgets setting and strategic financial planning. Management in collaboration with the PCN leadership and Leeds GP confederation banking service. Ensuring that claims are made appropriately via CQRS.
Provide project management and operational support for schemes such as the C-19 vaccine programme, seasonal influenza programme.
Lead of digital initiatives e.g PATCHs or online appointments, apps
Support practices and staff to optimise use of existing functionality and new technology to deliver benefits for patients and staff and enable improvement.
Use improvement techniques to focus on delivery of changes and analysis of their impacts, providing feedback to staff on both positive and negative impacts of changes made.
Use data to support population health management and workforce planning.
Advise on innovative opportunities and support practices to maximise benefits from innovation, through the sharing of learning and best practice.
IM&T:
Ensure the PCN is compliant with GDPR and ICO along with other relevant legislation and professional information management standards such as GMC/BMA and LMC guidance.
To ensure the integrity of the system working with the CCG to implement their guidance.
Manage PCN resources such as laptops, and other IT equipment.
Strategy and Leadership:
Co-development of a PCN strategic vision which will inform the choices we make in terms of clinical delivery.
Clarify objectives and priorities for network delivery in line with operational realities, financial parameters and contract requirements.
To deliver on decisions, priorities and business plans coming from the PCN board.
Promote the engagement and co-working of PCN GP practices
To build excellent relationships with the senior managers of the PCN GP practices
Support and champion the development of good quality locality network plans that deliver relevant change and improvement projects which can be both measured and evaluated.
Identify and develop appropriate tools and measures of improvement to support effective change, e.g. performance frameworks, population health data, outcome measures.
To be the PCN lead for Health Inequalities; identifying areas of need; project managing identified improvements.
To be the PCN representative at Local Care Partnership (LCP) meetings.
Welfare:
Organise PCN events e.g. Educational/Locality/Network wide events.
Relationship Management:
To act as a single point of contact to PCN staff including Clinical Director and Executive Team
Build strong relationships with all stakeholders including the CCG, Calibre, LCH, LCP, GP Confederation.
Provide coaching and support to staff and stakeholders involved in PCN transformation and development.
Represent the PCN at meetings, in the community and to the media as and when required.
Work closely with GPIT at ICS/ICB level to support digital transformation and initiatives.
Information and data management
The post holder will need to have skills in relation to understanding and using data to drive improvement and assess impacts. These skills will be used to:
Take a data driven approach, looking at evidence and information available to support understanding of challenges, identify possible solutions and track the impacts of changes that have been made.
Work in partnership with the ICS to identify relevant data sets and to support standardisation in the use of measures for understanding change and improvement in general practice.
Bring together data from a variety of sources and use information to develop insights into working practices, or support population health management, risk stratification or workforce planning, effectively tailoring content to meet the needs of the audience.
Effectively present and communicate data to practices and PCN staff at all levels to increase understanding of operational challenges
Interpret data and highlight variation to support open discussion about where variation is and is not warranted
Help PCN and practice staff understand the limitations of particular data sets and where other information may be available to supplement understanding of issues
Support and encourage a focus on data in understanding the effectiveness of changes made
Seek and secure analytical resource as required to support the PCN in developing and implementing appropriate data and information sharing mechanisms
Use data to track and assess impact of changes made to operational processes, and to track trends and developments over time.

The post-holder will participate in any training programme implemented by the PCN as part of their employment.
Carry out identified statutory and mandatory training.

Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work.
Quality and Governance:
The post-holder will strive to maintain quality within the PCN, and will: -
Alert other team members to issues of quality and risk.
Assess own performance and take accountability for own actions, either directly or under supervision.
Work effectively with individuals in other agencies.
Effectively manage own time, workload and resources.
Ensure that the PCN board and Clinical director adhere to the governance structure as set out in the network agreement.
Communication:
We would anticipate that the successful candidate will work across the five practices with the possibility of some home working
The PCN Development Manager will ensure effective communication is maintained within the Primary Care Network and with all stakeholders outside of the network.

- Communicate effectively with partnership groups, patients and carers.
