Job summary

Pike View - Dr Malhotra & Partners is a GP surgery located in the new Horwich Health & Wellbeing Hub with just over 10,500 patients.

The right candidate will help us to continue delivering safe and co-ordinated care to our patients with focus on chronic disease management, cytology, immunisations, contraception (pill/depo).

Main duties of the job

Implement and evaluate individual treatment plans for chronic disease patients.

Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health.

Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF.

Support the team in dealing with clinical emergencies.

Implement vaccination programmes for adults and children.

Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual).

Support the clinical team with all safeguarding matters, in accordance with local and national policies.

Deliver opportunistic health promotion where appropriate.

About us

Pike View Dr Malhotra & Partners is one of three practices that make up Horwich Primary Care Network (PCN). We have a small branch surgery located in Westhoughton.

We are long standing high achieving practice in QOF and BQC.

Our clinical system is EMIS alongside Docman 10, Ardens, Accurx, Iplato.

Due to move into a brand new purpose built A+ energy rated building, we have expanded and are expanding our clinical and reception/administrative team.

Job description

Job responsibilities

Develop, implement and embed health promotion and well-being programmes.

Implement and evaluate treatment plans and chronic disease patients.

Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects on the patients' health.

Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF.

Provide travel medicine services.

Request pathology results as required.

Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required.

Maintain accurate clinical records in conjunction with extant legislation.

Ensure read codes are used effectively.

Maintain Chronic disease registers.

Chaperone patients where necessary, prioritise health issues and intervene appropriately.

Support the team in dealing with clinical emergencies.

Support patients int he use of their prescribed medicines or over the counter medicines (within own scope of practice).

Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
