Job summary

We are looking for a Triage Co-Ordinator to help patients get the care they need. In this role, you will support the triage team by managing requests from patients and teams, making sure patients are directed to the right healthcare professional or service. You will help people access care for a wide range of physical, mental health, and social issues. This includes keeping admin lists up to date, supporting the triage team with daily tasks, and signposting patients to the most appropriate services. You will play an important part in helping people get the right support at the right time.

Main duties of the job

  • Manage patient requests, including routine and urgent triage assessments

  • Support patients by navigating them to the most appropriate care or service

  • Facilitate effective care pathways and ensure timely responses to patient needs

  • Act as a key point of contact for patients and staff within the triage team

  • Work closely with clinical and non-clinical colleagues, including the wider multidisciplinary team (MDT)

  • Liaise with patients to gather relevant information for clinical triage and offer signposting as guided by clinicians

  • Monitor and manage daily administrative triage lists to coordinate patient care and follow-ups

  • Assist in managing same-day appointment availability and support the smooth running of triage operations

  • Attend relevant meetings and contribute feedback on triage processes and patient flow

  • Support patients with long-term conditions, urgent issues, and access to wellbeing or social care services

  • Use IT systems extensively for record-keeping, communication, and managing patient pathways

  • Carry out general administrative tasks to support the triage and wider practice teams

The aim of this role is to facilitate effective triage pathways by managing requests and appropriately navigating patients in accessing health care services.

As a member of the Triage Team, you will play a critical role in helping patients access the right support at the right time. This involves managing administration lists and providing support to the team to help patients receive support for a wide range of physical, social and mental health issues.
