Job summary

We are looking for a Practice Nurse (Full or Part Time) to join our large, friendly GP practice. The right person will complement our forward-thinking nursing team working alongside GPs, Physician Associates, Clinical Pharmacists, a Social Prescriber, Drugs Counsellor, MSK physios, youth counsellor and mental health practitioner.

You will become a member of a supportive and professional nursing team (5 Practice Nurses and an Assistant Practitioner), benefit from an experienced group and become a core member of the Practice in providing effective and patient centric care. The practice is involved with the local PCN, giving you access to a wider team, training opportunities and a wider supportive network.

Main duties of the job

The practice nurse will work as part of the primary healthcare team providing Personal Medical Services to the patients of the practice; main responsibilities include:

  • Supporting clinical teams
  • Cytology
  • Wound care, removal of sutures, ear syringing and venepuncture
  • Chronic disease management
  • Routine adult and childhood immunisations
  • Maintaining and cleaning equipment. Maintaining GP & nurse rooms & stock ordering. Maintaining the cold chain log.
  • Infection control
  • Home visits as part of the annual flu vaccination programme
The nursing duties cover extended hours which include Saturdays , shared fairly among the team.

Job Summary

The practice nurse will be expected to provide care in the treatment room for the patients of Paxton Green Group Practice. This would normally be carried out by face-to-face consultations including, adult and childhood immunisations, dressings, ear syringing & core nursing services. The postholder will also be expected to work during extended hours & this will include Saturdays and weekday late evenings.

Areas of Clinical Responsibility

  • To carry out nursing care plans and treatments in conjunction with GPs or independently to agreed protocols.
  • To work in accordance with clinical guidelines and practice policies
  • To carry out health screening as required
  • To carry out all general treatment room duties including, dressings, ear syringing etc.
  • To carry out adult & childhood immunisations
  • Undertake NHS health checks and Holistic Health Assessments, participate in research projects, as & when appropriate.
  • To maintain appropriate records and statistics
  • To communicate any abnormal findings to other members of the nursing team and/or the GPs, as appropriate
  • To visit patients at home as and when appropriate
  • To be active in health promotion and patient education
  • To carry out specialist nursing activities as appropriate and to participate in training and development
  • To work within the principles of the Practices PMS contract. and Quality & Outcomes Framework (QOF) and the nGMS contract
  • Attending primary health care team meetings and other meetings as may be necessary for your duties

  1. To adhere to the NMC code (Nursing & Midwifery Council) of Professional Conduct and to work in accordance with the scope of professional practice
  2. To take responsibility for ones own professional practice and educational updates, in accordance with Prep and the needs of the practice
  3. Participate in activities that lead to personal and/or team development
  4. Participate in Development Reviews, as and when required

Support

We normally arrange for all new staff members to have a mentor or supporter within the practice team.

All staff have regular appraisals and have access to appropriate training and development.
