Job summary

Paxton Green Group Practice (PGGP) is seeking a Prescribing Clinical Pharmacist to join our clinical team. This is an exciting opportunity to be part of a large training practice with a learning ethos and involved in the Primary Care Network (PCN) collaboration with only two other of our neighbouring practices. You will be based full time at Paxton Green Group Practice, in F2F, phone & online consultation clinics. PGGP has approximately 19,500 patients and is a training practice for GP registrars, FY2 doctors, medical students, practice nurses, paramedics, CPPE pharmacists and foundation pharmacists (pre-reg).

The ideal candidate should be flexible with a proactive approach to work, be an independent prescriber, and with a track record of delivering patient facing clinics and demonstrably reducing workload for GPs and Practice Nurses. This will be tested at interview.

Main duties of the job

The post holder will:

  • Be a qualified Independent Prescriber
  • Supervise Pre-Reg pharmacists
  • Supervise Pharmacy technicians
  • Be a role model for CPPE pharmacists
  • See-Treat-Discharge F2F booked patients for minor ailments and LTCs independently.
  • Telephone and Online clinics
  • Divert the workload of routine prescriptions from the general practitioners
  • Work as part of a multi-disciplinary team in a patient facing role, in particular in chronic disease management, alongside practice nurses, in F2F booked clinics.
  • Resolve patient medication queries by phone on the medication advice line
  • Respond to patient queries received via PGGP web based forms
  • Undertake clinical medication reviews to proactively manage patients with complex polypharmacy
  • Provide support to general practice staff in relation to prescription queries
  • Provide clinical leadership on medicines optimisation
  • Disseminate MHRA and other alerts to the wider team
  • Support PCN Extended Access - Saturdays 9am-4pm in a fair share rota with colleagues, with F2F booked patientclinicse.g. NHS Health Checks, LTC reviews (9-care processes), HTN & Asthma.
  • Support PCN covid / flu vaccination clinics - both based on site at the practice and for housebound patients /care homes in a rota.

About us

Weekly working hours can be in the range 30 to 37.5 hrs / week over 4-5 working days. Salary is on PGGP clinical payscale grade I. We have a comprehensive benefits package compatible with Agenda for Change including the NHS Pension.

You will be supported to maintain and extend your professional practice with generous study leave, regular PCN and Borough wide training events and mentoring with a GP partner.

Croxted PCN has about 35,000 patients and has formed a cohesive network of high quality providers. The PCN has embraced the digital transformation of primary care and this role is an opportunity for an ambitious clinical pharmacist to develop their professional patient facing practice to its maximum potential, supervised by experienced trainer GPs and the PCN's clinical director.
