We are recruiting for a PCN Enhanced Practice Nurse to support general practice in Walsall and in particular South 2 PCN. The successful candidate will work as an autonomous practitioner, working independently but with supervision and with other health care professionals to assess, diagnose and treat patients with a range of acute, non-acute and chronic medical conditions. The postholder will deliver enhanced clinical care across several GP Practices within the South 2 PCN and will work from GP surgeries.

Main duties of the job

Specific duties include, working as part of a multi-disciplinary team within the PCN, consult with patients, families and the wider team to undertake assessments of patient need and devise and evaluate complex care plans. Evaluate and analyse clinical problems using clinical knowledge. Deliver enhanced clinical care in a contact of continual change. Teach and advise patient and families on managing their conditions or support the MDT to do so. Participate in clinical audits, research and implement changes. Develop and update practice protocols and local guidelines. Recognise and refer patients appropriately. Delegate work appropriately within the MDT using the principles of delegation. Demonstrate initiative and creativity in solving problems. Hold responsibility for team performance and service delivery. Provide supervision to trainee nursing associates, nursing associates and general practice nurse. Facilitate and support the development and delivery of innovative training and education to improve staff competence and confidence. Lead and/or participate in formal training events promoting best practice. Provide professional and clinical leadership and mentorship, acting as a role model for evidence-based practice. Support the development and delivery of PCN work streams aligned to areas of clinical expertise. Work proactively with stakeholders to develop integrated care pathways and holistic models of care.
