We are recruiting for an Advanced Clinical Practitioner to support general practice in Walsall and in particular, the PCN and its objectives. The successful candidate will work as an autonomous practitioner working independently and with other health care professionals to assess, diagnose and treat patients with a range of acute, non-acute and chronic medical conditions, initiating treatment as appropriate. The post includes working within the Walsall practices.

The post will work with all the 5 clusters of GP surgeries known as Primary Care Networks across the North, East, South or West of Walsall. The post will be based at and provide the service from the GP surgeries, with some requirement to attend team meetings and training at OurNets Head Office, located in Bloxwich, as well as training at other local or regional venues as necessary.

ACPs:

Candidates will have a masters degree. Is working at a masters level aware or equivalent that encompasses the four pillars of clinical practice, leadership and management, education and research, with demonstration of core capabilities and area specific clinical competencies. Has qualified from an approved 18-month training pathway or equivalent and qualified as an independent prescriber

Main duties of the job

To work as an autonomous practitioner working independently and with other health care professionals to assess, diagnose and treat patients with a range of acute, non-acute and chronic medical conditions, initiating treatment as appropriate.

To assess, diagnose and treat undiagnosed and diagnosed conditions for patients within a primary care setting.

To develop innovative concepts, models, methods and practices which deliver new and improved primary care services to meet the changing needs of primary care.

To trial and adopt new working methods designed to manage the increasing demand in primary care.

To provide education and training to other staff and students as appropriate and to take a lead role in encouraging teamwork and sharing best practice.

To undertake audit and research if and when required.

To promote healthy lifestyles, health promotion and disease prevention, by suggesting and recommending possible solutions.

Manage caseload of patients with common/minor/self-limiting ailments while working within a scope of practice and limits of competence

Ensure that patients are referred to the appropriate healthcare professional for the appropriate level of care within an appropriate period of time eg. Pathology results, common/minor ailments, acute conditions, long term condition reviews, etc.

OurNet Health Services is a provider of primary care service to NHS patients in Walsall. Established in 2019 with the support of GPs in Walsall, we aim to deliver excellent care to patients when they need it most.
