Job summary
We are recruiting social prescribers to support individuals to access the
support of voluntary and community organisations and make social connections.
The role, often referred to as social prescribing, will involve:
working with GPs and other health and care professionals to identify those
who would benefit most from support within their local community,
supporting people to create their own wellbeing plan, focused on
non-clinical aspects of mental and physical wellbeing, and supporting them to
engage with local community activity to meet their needs and desires,
Each post will work with one of five clusters of GP surgeries known as
Primary Care Networks across the north, east, south or west of Walsall. The
posts will be based at and provide the service from the GP surgeries or other
locations, with some requirement to attend team meetings and training at
OurNets Head Office, located in Bloxwich, as well as training at other local
or regional venues as necessary.
The ideal candidate will:
have experience of working with people with complex social and health
needs.
be committed to supporting individual decision making and a person-centred

have experience of working with voluntary and community organisations.
This role is fixed term for 12 months subject to funding arrangements.
Main duties of the job
Working closely with Primary Care Network Clinical Directors and take
direction from them as to the needs of the service.
To work alongside clinical and non-clinical staff within GP surgeries to
identify and receive referrals for individuals who would benefit from support
within the community to address their non-clinical needs.
To support individuals to undertake a holistic self-assessment, based on
the persons priorities, and co-design a simple personalised care and support
plan, identifying support needs to ensure maximum engagement in improving
health and wellbeing and enable individuals, and their families and carers to
live independently and improve their health outcomes
To support individuals to access service and activities and make
connections within their community and address their needs and desires as
identified in their Personalised Care Plan.
To provide individuals with a coordinated service, remaining point of
contact throughout as required.
To attend and input into multi-disciplinary team meetings as required to
develop and inform individual care plans.
To undertake follow up assessments with individuals at set intervals and
according to agreed review timescales, as required.

In Walsall, networks of local GP practices have formed seven Primary Care
Networks (PCNs) to help improve the care offered to the local population and
develop new ways of working. One of those opportunities is the role of Primary
Care Pharmacy Technician. This role will involve supporting clinical and
non-clinical practice staff across the network in relation to aspects of
medicines and their impact. The successful candidates will work closely with
GPs, Clinical Pharmacists, Nurse Practitioners, Practice Nurses, Community
Nurses and other healthcare professionals to promote safe, effective and high
quality, cost effective use of medicines in prescribing decisions.
