Job summary

We are recruiting Paramedics to support general practice in Walsall and in particular, the PCN and its objectives. The successful candidate will work as an autonomous practitioner working independently and with other health care professionals to assess, diagnose and treat patients with a range of acute, non-acute and chronic medical conditions, initiating treatment as appropriate. The post includes working within the Walsall practices and completing home visits to patients both in their own homes and the community setting.

The post will work with all the five clusters of GP surgeries known as Primary Care Networks across the North, East, South or West of Walsall.

The post will be based at and provide the service from the GP Surgeries with some requirement to attend team meeting and training at OurNet's head office located in Bloxwich as well as training at other local for regional venues as necessary.

The ideal candidate will have experience in working minor illness clinics or have relevant experience in assessing patients and will be registered with the HCPC.

Main duties of the job

Working closely with Primary Care Network Clinical Directors and take direction from them as to the needs of the service. To support the PCN GP practices with home visiting workload.

To work as an autonomous practitioner working independently and with other health care professionals to assess, diagnose and treat patients with a range of acute, non-acute and chronic medical conditions, initiating treatment as appropriate.

The proactively manage housebound/care home patients to increase the quality of care provided to this vulnerable group of patients.

The assess, diagnose and treat undiagnosed and diagnosed conditions for patients within a primary care setting.

To utilise telephone triage for all home visit requests were appropriate.

To collaborate with GPs, Nursing staff and non clinical support staff.

Use their individual skill and experience in order to make a positive contribution to the organisation.

To develop innovate concepts, model, methods and practises which deliver new and improved primary care services to meet to the changing needs of primary care.

To trial and adopt new working methods designed to manage the increasing demand in primary care.

To provide healthy lifestyles, health promotion and disease prevention, in particular to housebound patients.
