Job summary

We are recruiting social prescribers to support individuals to access the support of voluntary and community organisations and make social connections. The role, often referred to as social prescribing, will involve:

working with GPs and other health and care professionals to identify those who would benefit most from support within their local community,

supporting people to create their own wellbeing plan, focused on non-clinical aspects of mental and physical wellbeing, and supporting them to engage with local community activity to meet their needs and desires,

Each post will work with one of five clusters of GP surgeries known as Primary Care Networks across the north, east, south or west of Walsall. The posts will be based at and provide the service from the GP surgeries or other locations, with some requirement to attend team meetings and training at OurNets Head Office, located in Bloxwich, as well as training at other local or regional venues as necessary.

The ideal candidate will:

have experience of working with people with complex social and health needs,

be committed to supporting individual decision making and a person-centred approach,

have experience of working with voluntary and community organisations.

This role is fixed term for 12 months subject to funding arrangements.

Main duties of the job

Working closely with Primary Care Network Clinical Directors.

To work alongside clinical and non-clinical staff within GP surgeries to identify and receive referrals for individuals who would benefit from support within the community.

To support individuals to undertake a holistic self-assessment.

To support individuals to access service and activities and make connections within their community and address their needs and desires as identified in their Personalised Care Plan.

To attend and input into multi-disciplinary team meetings as required to develop and inform individual care plans.

To undertake follow up assessments with individuals at set intervals and according to agreed review timescales, as required.

To keep accurate records using agreed systems and contribute to programme evaluation.

To work according to confidentiality and data sharing policies of OurNet Health Services Ltd, the Walsall PCNs and its relevant partners.

Manage and prioritise their own case load, in accordance with needs, priorities and any urgent support required by individuals on the case load.

Where required and appropriate refer people back to other health professionals within the primary care network.

Support staff within the primary care network with information about local voluntary and community resources and how patients can access them.

At OurNet we have an enthusiastic team of clinicians and administrators. There role will contribute to improving the quality of care for our patients across the borough.

Qualifications

Essential

  • Educated to at least Level 3 in a health, social care, counselling
  • or other relevant field, or suitable relevant work experience.
  • Excellent communications skills and experience of providing empowering support to adults in a planned and structured way to improve health, recovery and well-being outcomes.
  • Experience of working within a health or care organisation (including mental health).
  • Skills in collating information and data on community resources and organising these into an accurate record to support a range of different agencies.
  • Excellent record keeping skills.
  • A confident and professional approach to working with a variety of stakeholders.
  • A compassionate and person-centred approach.
  • Ability to work on own and within a team.
  • A proven understanding of safeguarding for children and vulnerable adults and ability to implement relevant policies and procedures.
  • The ability to work autonomously and to plan, prioritise work under pressure and adapt to new models of working.
  • Experience of building positive relationships to encourage networks of organisations that support collaborative working.
  • Computer software programmes including the Microsoft Office suite and bespoke officer software
  • Full UK Drivers Licence and access to a vehicle for work purposes.
  • Flexibility and availability to work from a range of bases in Walsall and for occasional out-of-hours working.
