Job summary
Registered nurses working at an enhanced level are expected to be able to manage discrete activities in complex, challenging and changing situations and environments. They should also be confident to seek further guidance when they reach the boundaries of their competence.
Main duties of the job
- Works as a part of a multi-disciplinary team within thePCN
- Consults with patients, their family, and the multi-professional team to undertake assessment of patient needs and devise and evaluate complex plans.
- Utilises clinical knowledge to critically analyse problems to seek out evidence to apply, utilise enhanced techniques, intervention, and equipment to make clinical decisions.
- Delivers enhanced Clinical care in the context of continual change, challenging environments, different models of care delivery, innovation and rapidly evolving technologies using analysis and their underpinning knowledge to manage complex interventions.
- Teaches and advises patient and their families on how to manage their condition or support the multi-disciplinary team (MDT) to do so.
- Works within, and recognises own scope of practice, understanding when and to whom patients should be referred using the principles of delegation, delegates work to other members of the MDT and take responsibility for the delegated activity as appropriate.
- Demonstrates initiative and are creative in finding solutions to problems.
- Undertake frailty assessments, chronic disease management reviews, and care home reviews, including home visits for patients with complex or mobility-limiting needs.
- Identify and refer appropriate patients to the pharmacy team for Structured Medication Reviews.
- Signpost and refer patients to social prescribing and physiotherapy services to support holistic, person-centred care.

GP practices have been working together in our area for years and formed BridgedalePrimary Care Network (PCN).Bridegale Primary Care Network is made up of GP registered patient lists for Roslea Surgery, Riverside Medical Centre, Ryan Medical Centre, New Longton Surgery and Lostock Hall Medical Centre. We serve a community of around 30,000 people. We are small enough to provide the personal care valued by our people and large enough to have impact and economies of scale through better collaboration.
As a Primary Care Network we are working hard to deliver on the national NHS England network contract that commenced in July 2019 and continues to March 2024.
This includes the following areas of focus:
anticipatory care
cardiovascular disease prevention and diagnosis
enhancing health in care homes
early diagnosis of cancer
tackling neighbourhood inequalities
structured medication reviews
social prescribing reviews
So as a primary care network ,with its member practices working ever closer together, it is important to establish shared core values which not only enables us to adapt, but to do so in the way that we all feel is right for our patients and staff. The outstanding contributions from Ryan, Roslea and Riverside surgeries in developing Bridgedales SHINE values is a key milestone and prepares the ground for the future.
