for x2 GP Assistants to join our PCN. The successful applicants will work
alongside the PCN team to support healthcare professionals, patients and
carers.
You will be part of an innovative multidisciplinary
team working across the Bridgedale PCN to support and assist the GP practices
within the PCN.
We are looking for dynamic and pro-active GP
Assistants on a fixed term 6 month contract to assist in improving patient outcomes and support the primary care
workforce in general practice.

Main duties of the job
Our
PCN is looking for GP Assistants to join our growing primary care workforce.
The successful candidates will work across the patch, within the PCN Practices,
Community Teams and across the Multi-Disciplinary Team (MDT).
GP Assistants will deal with patients and, if appropriate, their carer, before
or after the patients consultation with a clinician or other healthcare
professional.
The
GP Assistants role requires them to be able to work with, and understand the
roles of, a variety of different people working in the practice and across the
PCN including doctors, nurses, healthcare assistants, social prescribing link
workers, physiotherapists, paramedics, health and wellbeing coaches, and
pharmacy technicians.
A
GPA will support the GP(s) smooth running of clinics by performing the more
routine administration and clinical tasks on behalf of the GP freeing up their
time to focus on the patient.
About us
GP practices have been working together in our area for years and formed BridgedalePrimary Care Network (PCN).Bridegale Primary Care Network is made up of GP registered patient lists for Roslea Surgery, Riverside Medical Centre and the Ryan Medical Centre. We serve a community of around 30,000 people. We are small enough to provide the personal care valued by our people and large enough to have impact and economies of scale through better collaboration.
As a Primary Care Network we are working hard to deliver on the national NHS England network contract that commenced in July 2019 and continues to March 2024.
This includes the following areas of focus:
anticipatory care
cardiovascular disease prevention and diagnosis
enhancing health in care homes
early diagnosis of cancer
tackling neighbourhood inequalities
structured medication reviews
social prescribing reviews

Signposting some post to others such as clinical
pharmacist etc.
Extracting all information from clinical letters that
needs coding and adding to notes.
Arranging appointments, referrals, tests and follow up

Preparing patients prior to going in to see the GP,
taking a brief history and basic readings in readiness for the GP appointment.
Dipping urine, taking blood pressure, ECGs &
phlebotomy
Completing basic (non-opinion) forms and core elements
of some forms for the GP to approve and sign such as insurance forms, mortgage,
benefits agency forms etc.
Explaining treatment procedures to patients
Helping the GP liaise with outside agencies e.g.,
getting an on-call doctor on the phone to ask advice or arrange admission while
the GP can continue with their consultation(s)
Support the GP with immunisations/wound care.
