Job summary

We are looking for a motivated and passionate Practice Nurse with strong diabetes management experience to join our team. In this role, you will lead and support highquality diabetes care within the practice, delivering personalised assessments, structured reviews, and ongoing clinical management for patients with Type 1 and Type 2 diabetes.

You will run diabetes clinics, interpret results, develop evidencebased care plans, and provide expert guidance on lifestyle management, medication optimisation, and complication prevention. Working alongside our GPs and wider multidisciplinary team, you will play a key part in improving health outcomes, empowering patients to selfmanage their condition, and contributing to practicewide quality improvement initiatives.

Main duties of the job

Deliver high-quality nursing care including:

Chronic disease management (Inc Diabetes Management and insulin initiation)

Cervical cytology and womens health clinics

Childhood and adult immunisations

Travel health advice and vaccinations

Wound care

Contraceptive services (e.g. pill checks, injections)

Coordinate administration of under Patient Group Directions (PGDs) and within scope of practice

Provide health promotion and lifestyle advice (e.g. smoking cessation, weight management)

Support patients in managing long-term conditions and medication adherence

Maintain accurate and timely clinical records using appropriate coding

Trinity Medical Centre is a well established and busy GP practice located in the heart of Wakefield, serving a large and diverse patient population from its Thornhill Street site. The practice provides a wide range of high quality primary care services, including chronic disease management, child immunisations, diabetes care, minor surgery, contraceptive services (including LARC), COPD and asthma clinics, and learning disability health checks.

As a forward thinking practice, Trinity Medical Centre also supports digital access, offering online triage systems, appointment booking, repeat prescription services, and remote medical advice to help patients receive timely, safe, and effective care.
