Job summary

We are seeking a dedicated, compassionate Practice Nurse to join our excellent team. Whether you are a New to General Practice Nurse within your first 2 years of qualifying or an experienced Practice Nurse looking for a new role at an excellent practice. The ideal candidate will play a crucial role in providing high-quality patient care, performing cervical smears, delivering essential vaccinations and assisting with chronic illness management, working collaboratively within the nursing team and wider clinical teams. This position requires a commitment to delivering exceptional patient care within a busy, professional, yet friendly environment.

Main duties of the job

Responsibilities

  • Conduct patient assessments and record medical histories accurately.
  • Administer vaccinations and medications as prescribed by physicians.
  • Monitor patients' vital signs and report any significant changes to the healthcare team.
  • Cervical Cytology
  • Assist in the management of chronic diseases and provide health education and disease prevention to patients.
  • Maintain accurate patient records within EMIS in compliance with confidentiality regulations.
  • Collaborate with doctors and other healthcare professionals to ensure comprehensive patient care.
  • Participate in health promotion activities within the community.

Experience

  • A recognised nursing qualification is essential, along with current registration with the relevant nursing body.
  • Previous experience in a clinical setting is essential primarily, in primary care or general practice.
  • Strong knowledge of anatomy and physiology is required to provide effective patient care.
  • Excellent communication skills, both verbal and written, are necessary for interacting with patients and colleagues.
  • A compassionate approach to patient care is vital, along with the ability to work well under pressure.

About us

We are an exceptionally friendly and supportive practice with high rates of staff retention. Our patient list size is circa 13500 and we employ 30 personnel.

The practice team is made up of five Partners, three Salaried GPs, two ARRS GPs, three Retainer GPs, two practice nurses, 2 HCAs, management team and admin staff. We are a training practice and regularly have a GP Registrar and medical students.

Wimbledon Village Surgery is a purpose-built medical centre in the heart of the community of Wimbledon Village and situated close to Wimbledon Common and the Wimbledon tennis club. Onsite parking is available

We have a good CQC rating and are part of North West Merton PCN and enjoy working closely with them in our locality. We have a PCN Pharmacy team, Social Prescribers, Physiotherapists and a Mental Health Practitioner

We work hard to deliver the best levels of care to our patients and pride ourselves on delivering excellent care, with kindness. Many of our patients have been with the surgery for decades and we regularly receive positive feedback from patients both long standing and newly registered.

Job description

Job responsibilities

The following are the core responsibilities of the Practice Nurse in delivering health services:

a. Support the Infection Prevention Control (IPC) lead and work to the IPC Policy

b. Consult with patients and their families to undertake assessments of patient need

c. Support in the long-term condition management within the organisation, including being involved with quality improvement and assurance initiatives

d. Develop, implement and embed an effective care management programme for the frail and housebound patients registered with the organisation

e. Support public health and screening programmes, including immunisations, vaccinations, and cervical screening and also provide opportunistic health promotion advice

f. Embed population health management approaches within general practice, targeting health inequalities and improving access

g. As required, provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF

h. Support new-to-practice nurses and other healthcare professionals within the organisation

i. Teach and advise patients and their families on how to manage their condition

j. Participate in clinical audits and research projects and implements changes as required, including supporting with the development, and updating of practice protocols/guidelines and procedures locally

k. Work within national and local protocols where these exist

l. Recognise the boundaries of their practice and know when and to whom patients should be referred

m. Support in the delivery of enhanced services and other service requirements

n. Be aware of duties and responsibilities regarding current legislation and adhere to practice policies and procedures on Safeguarding Adults and Safeguarding Children

o. Participate in the management of patient complaints when requested to do so and participate in the identification of any necessary learning brought about through incidents and near-miss events

p. Undertake all mandatory training and induction programmes

q. Contribute to and embrace the spectrum of clinical governance

r. Support the practice team with the reviewing and implementation of practice policies and protocols, ensuring conformance to extant legislation

s. Attend a formal appraisal with their manager at least every 12 months. Once a performance/training objective has been set, progress will be reviewed on a regular basis so that new objectives can be agreed

t. Lead on public health campaigns (e.g., flu clinics)

u. Provide travel advice services

v. Provide wound care (ulcer/Doppler, etc.) to patients

w. Maintain accurate clinical records in conjunction with extant legislation ensuring that SNOMED CT codes are used effectively

x. Contribute to practice targets (QOF, etc.), complying with relevant guidance

y. Maintain chronic disease registers and implement an effective call/recall system

z. Maintain a clean, tidy, effective working area at all times

aa. Chaperone patients where necessary

bb. Support the team in dealing with clinical emergencies

cc. Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients, etc.)
