Job summary
Chorleywood Health Centre has an exciting opportunity available for an
Advanced Nurse Practitioner to join our medical team and work as part of a
multi-disciplinary team.
Main duties of the job
You will be responsible for clinical assessment, treatment and / or referral or giving advice as appropriate to both adults and children presenting with a wide spectrum of clinical conditions.
You will be a Registered General Nurse; have broad, autonomous practice experience and can use your advanced clinical examination skills within general practice, utilising advanced clinical skills to assess, diagnose, treat and refer patients requiring relevant diagnostic investigations or for advice of relevant specialities. In addition, you will organise and coordinate the provision of nursing services for the Practice to include the delegation of duties to other members of the Practice nurse team as appropriate.
You will have excellent non-verbal and verbal communication skills when dealing with patients and their carers, have the ability to make clear decisions with confidence and communicate these effectively.
You will have excellent Team skills and be able to demonstrate experience of multi-disciplinary team working.
About us
Chorleywood Health Centre was founded in 1926 and absorbed into the NHS in 1948.
The health centre is set in a purpose-built building located in the village of
Chorleywood, Hertfordshire, on the
border with Buckingham. It has excellent transport links being close to the M25
(junction J18) as well as the tube and overground lines.
The practice has a multidisciplinary
team which includes:

Acute illness
- Run acute illness clinics
- Ensure clinical practice is safe, effective and remains within boundaries
of competence and that limitation are acknowledged
- Advise patients on general health care and minor ailments, with referral
to other members of the primary and secondary health care team when
necessary
Nursing Team- Organise and coordinate the provision of nursing services for the
Practice to include the delegation of duties to other members of the
Practice nurse team as appropriate
Chronic Disease- Assist with the running of chronic illness clinics including diabetes,
asthma, COPD and ensure evidence-based practice as suggested by
the Quality and Outcomes Framework (QOF) and other national and local services

Pathological specimens and investigatory procedures- Undertake the collection of pathological specimens including
intravenous blood samples, swabs etc. Perform investigations
requested by the GPs
Administration and professional responsibilities- Participate in the administrative and professional responsibilities of the
Practice team
- Ensure accurate, contemporaneous notes of all consultations and
treatments are recorded in the electronic patient record (EPR)
- Ensure accurate completion of all necessary documentation associated
with patient health care and registration with the Practice
- Ensure collection and maintenance of statistical information required
for research, teaching and audit
- Attend and participate in Practice meetings
- Restocking and maintenance of clinical areas and consulting rooms
- Participate in the teaching of medical students and postgraduate
Registrars
Minor Operations- Management of the treatment room for minor operations including
maintenance of equipment and stock.
- To assist the doctors to carry out minor operations as required
- Report any problems to the Practice Manager
Supplies and equipment- Maintenance of equipment and stock relating to patient care
Training and personal development- Training requirements will be assessed by yearly appraisal and will be
in accordance with Practice requirements. Personal development will
be encouraged and supported by the Practice. It is the individuals
responsibility to remain up to date with recent developments
- Participate in the education and training of students of all disciplines
and the Introduction of all members of the Practice staff where appropriate
- Maintain continued education by attendance at courses and study days
as deemed useful or necessary for professional development ensuring
PREP requirements are met.
- If it is necessary to expand the role to include additional
responsibilities, full training will be given
- Develop and maintain a Personal Learning Plan
Liaison- As well as the nursing team there is a need to work closely with
reception, office to ensure the smooth running of the Practice, reporting
any problems encountered to the relevant person and ensuring
everyone is aware of the different roles within the nursing team.
- There is also the need to establish and maintain good liaison with other
local practices, including the primary care network as well as other agencies including secondary care

- It will be necessary to attend and contribute to various Practice
meetings
Confidentiality
- Ensure the maintenance of patient confidentiality as per the Practice
protocol. Essentially, information relating to patients, carers,
colleagues, other healthcare workers or the business of the Practice
may only be divulged to authorised persons in accordance with the
Practice policies and procedures relating to confidentiality and the
protection of personal and sensitive data
