Job summary
The Helsby and Elton Practice
are currently recruiting for a professional, caring and hardworking practice
nurse with a specialist interest in diabetes to join our friendly and
supportive nursing team. The ideal candidate will have experience of working as
a practice nurse managing diabetes within primary care with a good
understanding of the Quality Outcomes Framework.
Please note this is a part time role with a minimum of 2 sessions per week and flexibility with hours can be considered.
Main duties of the job
The successful candidate will
provide and maintain a high standard of nursing care for patients, focussing
on, but not limited to, the monitoring and management of diabetes. This will
include both patient education and pharmacological intervention for diabetes
management. This role will be supported by the diabetes lead nurse. Other
duties may involve immunisations, cervical screening, long term condition
management such as hypertension however experience within these areas is not
essential.

established practice of 9900 patients from a diverse range of social, ethnic
and cultural backgrounds. Our team currently consists of 5 GP Partners, 2
salaried GPs, 3 Advanced Nurse Practitioners, 2 Practice Nurses, 1 GP Assistant
and 2 Healthcare Assistants, as well as a bank of reception and admin staff who

Job responsibilities
The main aspect of this role is for diabetes management. This will involve routine monitoring, annual reviews, follow ups/ diabetes queries. This will involve working within local and national guidelines to provide excellent high quality care to our patients living with diabetes. Although prescribing is not essential it will involve decision making around pharmacological intervention with support from the diabetes lead nurse/GPs as required.
Although the role is not advertised for general practice nurse duties, it may involve the following:
- Cervical Screening
- Routine Immunisation/Childhood Immunisation
- LTC management (hypertension, stoke, IHD)
- requesting laboratory tests such as bloods, urine sample, swabs (as appropriate)
- Urinalysis
- Administration of injections such as B12, Depo Provera, Testosterone injections
