We are offering an exciting opportunity for a motivated & experienced Advanced Nurse Prescriber or Advanced Clinical Practitioner to join our friendly & well established team, working in a successful group of 4 general practices in and around the Walsall area. We have recently merged 3 of our practices into one, working out of a brand new, purpose built facility in the very heart of Walsall.
The main purpose of post:
As a practitioner
within a clearly structured and supported clinical team you will be responsible
for delivering high quality care, demonstrating skills in critical thinking and
clinical decision making. You will share in the clinical and administrative
workload of the organisation, undertaking minor illness and chronic disease
management clinics, medication reviews and duties of pathology and repeat prescriptions. You will be
supported in this role by our GPs and existing Nurse
Prescribers & Pharmacists. Applicants will have
completed the Independent Prescribing course and be confident working
autonomously, as well as under the guidance of our GPs.
Main duties of the job
To
work as an autonomous practitioner working independently and with other health
care professionals to assess, diagnose and treat patients with a range of
acute, non-acute and chronic medical conditions, initiating treatment as
appropriate.
To
undertake telephone triage and advice calls.
To
visit patients either in their own home or in a care home environment.
To
collaborate with GPs, nursing staff and non-clinical support staff.
Use
his/her individual skill and experience in order to make a positive
contribution to the organisation.
To
develop innovative concepts, models, methods and practices which deliver new
and improved primary care services to meet the changing needs of Umbrella
Medical.
To
trial and adopt new working methods designed to manage the increasing demand in
primary care.
To
provide education and training to other staff and students as appropriate and
to take a lead role in encouraging teamwork and sharing best practice.
To
undertake audit and research if and when required.
