Job summary
We are currently looking for an experienced Practice Nurse
to join our team at Ashmore Park Medical Centre. This is a full - time position
(37.5hrs per week). We are looking for someone who is highly motivated,
enthusiastic and committed to join our team.
Main duties of the job
Develop, implement, and embed health promotion and wellbeing
programs
Create and evaluate individual treatment plans for both
acute and non-acute conditions
Support the development of individual treatment plans for
patients with chronic diseases
Identify and manage patients at risk of long-term
conditions, preventing adverse health effects
Provide routine nursing care in accordance with
clinical-based evidence, NICE, and the National Standards Framework
Support patients in compliance with prescribed medications
Request pathology services as necessary and interpret
results
Maintain accurate clinical records / personal care plans,
using effective SNOMED codes
Maintain chronic disease registers
Prioritise health issues and intervene appropriately
Recognise, assess, and refer patients with mental health and
community health needs
Implement vaccination programs for adults and children
Liaise with external services/agencies
Support the clinical team with safeguarding matters,
following local and national policies
Conduct a comprehensive physical health assessment of
patients with complex multiple healthcare needs
Interpret results of different assessments to make diagnosis
and plan to deliver care
Work independently or as part of a multi-disciplinary team

About us
The practice has a very experienced, stable team, a single -
handed GP, a part-time Advanced Clinical Practitioner, Health Care Assistant, a
Practice Manager and a team of receptionists/administrators. We are also
supported by various ARRS staff provided by the PCN to which we belong.
The practice is rated as 'good' by the CQC, is a high 'QOF'
achiever.
