West Walk Surgery have an exciting opportunity for someone to join our caring and professional Care Navigator team working 18 hours a week.
Days to be discussed at interview but you must be available for one 7.30am start and either a 8pm finish on a Monday or a 6.30pm finish on a Thursday.
We also ask that you are able to offer holiday/sickness cover where needed.
Main duties of the job
We are looking for a happy, caring person to assist our patients in being directed to the correct clinician for advice/treatment working alongside our GP led KLINIK team.
You will be Care navigating by answering phone calls, dealing with patients face to face, sending messages to patients and answering queries sent through our clinical system KLINIK.
We have an established, friendly Care Navigator team waiting to welcome and train you.

West Walk Surgery is a large friendly practice, consisting of 5 GP Partners, 5 salaried GP's, Mental Health Nurse, Pharmacy team, Paramedics, Nurse Practitioner, Practice Nurses and HCA's, all supported by the Care Navigator and Admin teams.

out swiftly. To assist in the provision of a pleasant working environment for

work at both the Yate and Coalpit Heath branch surgery.
Duties and
Responsibilities:
Work with our GP led KLINIK team.
Make
future appointments for patients with doctors, nurses and other
clinicians.
Navigate
patients to the most suitable person to deal with their problem.
Provide
general information such as results of tests carried out.
Register
regular, temporary, immediately necessary or emergency patients using the
approved guidelines.
Take
visit requests according to guidelines, ensuring you have obtained the patients
name, address, telephone number and message.
General
reception administration
When
receiving patient samples, record on computer and take to treatment room.
When
required chaperone a doctor when they are examining a patient.
Process
sick note certificate requests.
Amendments
and deductions
Process
incoming and outgoing medical requests.
Liaise
with outside agencies and patients in connection with medical report requests.
Have
a good working knowledge of all office equipment and use as required and
instructed.
Respond
to the call assist system as per the guidelines.
Use
the panic button as per the guidelines.
Attend
practice meetings as required.
